Key Responsibilities

Collaborate with interdisciplinary home care team members (nursing, OT, speech, social work)
Communicate effectively with physicians regarding patient status and progress
Maintain compliance with state, federal, and organizational regulations
Support patient independence, safety, and overall quality of life
Qualifications
Graduate of an accredited Physical Therapy program
Active Physical Therapist license in the state of Nebraska
Home health or outpatient experience preferred (new grads welcome)
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills
Ability to work independently and manage a flexible schedule
Valid driver's license, reliable transportation, and current auto insurance
CPR certification (or willingness to obtain)
