This role is responsible for assessing, planning, implementing, and evaluating physical therapy care to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. Minimum Qualifications: Current, active Physical Therapist (PT) license in the state of California. Minimum of one (1) year recent clinical experience, preferably in home health or acute care. Knowledge of and ability to comply with Title 22, Medicare Conditions of Participation, and ACHC standards. Current CPR certification. Valid California driver's license, reliable transportation, and proof of auto insurance. Strong clinical assessment, communication, and documentation skills. Ability to work independently while managing multiple patients and priorities.
