Reporting Relationship Director of Patient Care Services/Nurse Manager Scope of Supervision None Responsibilities include the following: Provide skilled physical therapy services to patients in the home setting, including evaluation, treatment, and patient/caregiver education. Develop, implement, and revise individualized therapy plans of care in collaboration with the interdisciplinary team and the patient's physician. Document all patient visits, assessments, and interventions in accordance with Medicare, Title 22, ACHC, and agency standards for accuracy, timeliness, and completeness. Educate patients and caregivers regarding safe and effective use of equipment, exercise programs, mobility training, and strategies to maximize independence. Monitor patient progress and communicate changes in condition or plan of care to the physician and interdisciplinary team promptly. Adhere to all agency policies, procedures, and safety protocols while promoting quality patient care and regulatory compliance. Participate in case conferences, staff meetings, and ongoing training as required. Submit all clinical documentation within 48 hours of each patient visit. Maintain professional knowledge and skills through continuing education and training in accordance with licensure and agency requirements. Minimum Qualifications: Current, active Physical Therapist (PT) license in the state of California. Minimum of one (1) year recent clinical experience, preferably in home health or acute care. Knowledge of and ability to comply with Title 22, Medicare Conditions of Participation, and ACHC standards. Current CPR certification. Valid California driver's license, reliable transportation, and proof of auto insurance. Strong clinical assessment, communication, and documentation skills. Ability to work independently while managing multiple patients and priorities.
