Day shift schedule with 5x8 hour shifts, 8:00 AM to 4:30 PM
Caseload includes up to 6 patients per day with 1:1 patient visits and 30 productivity points standard
No weekend, on-call, or floating to like units required, but may be expected to float to other facilities within the state
Epic EMR and Oasis documentation required, with use of Epic Remote Client (laptop) and Rover (phone app)
Travel radius may occasionally extend up to approximately 60 miles or 60-90 minutes of drive time daily
Business casual attire is required; scrubs are permitted but not required with no specific color
Responsibilities include patient assessment, developing individualized care plans, and providing skilled therapy services in patients' homes
Mileage is reimbursed at the IRS standard rate
All travelers must be willing to float between facilities within the state if required
Modules and online training are completed pre-start and annually, averaging 16 hours
First-time travelers are welcome to apply
Former permanent staff must wait at least 3 months before returning as a traveler and cannot be within 50 miles of the facility
Minimal to no RTO is preferred; extended or multiple days off may impact assignment consideration
Free parking is available at all work sites
Requirements

About the Opportunity
We are seeking a licensed, patient-focused Physical Therapist (PT) to join a quality-driven healthcare team. This role offers the flexibility to work in a variety of clinical environments based on your experience, preferences, and career goals.

What You'll Do
Evaluate and treat patients based on individualized plans of care
Provide skilled, patient-centered physical therapy services

Provide timely and accurate documentation following state and/or insurance guidelines
Requirements
Must have 6 months of experience; however, new grads are encouraged to apply. Must have BLS and state-specific licensing.
Physical Therapists should be able to stand and walk for long periods of time, as well as bend, lean or stoop without difficulty. PTs must have a sound knowledge of anatomy, physiology, pathology and medical terminology.
