Participates in the implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning. Incorporates patient care goals established in the Plan of Care, into therapy care, as evidenced by documentation in therapy notes. Performs occupational therapy services in accordance with accepted standards of practice and certified by the patient's physician. Instructs and provides patients and/or their families with “activities of daily living” utilizing adaptive equipment as well as sensory integration and fine motor coordination skills. Assists patient and/or caregiver in securing necessary equipment, which may include fabrication of custom- made splints. Instructs the patient and/or caregiver in the therapy home program providing modification and clarification when necessary. Counsels and instructs the patient and family in meeting therapy and related needs. Supervises and/or provides clinical direction to the Certified Occupational Therapy Assistant (COTA), Home Health Aide (HHA), and clinical staff to ensure the therapy care plan is followed and quality of care is being provided. Assist the Infection Control Coordinator in the development, implementation, and revision of written aseptic and isolation techniques. Ensure that therapy personnel performing tasks that involve potential exposure to blood/body fluids participate in appropriate in-service training programs prior to performing such tasks. Ensure that adequate supplies of personal protective equipment are on hand and are readily available to personnel who perform procedures that involve exposure to blood or body fluids. Ensure that therapy personnel follow established infection control procedures and universal precautions when performing duties. Care Plan Functions Assist in developing occupational therapy plans for individual residents in coordination and conjunction with the resident assessment (MDS). Review the occupational therapy requirements of each resident admitted to the facility and assist the attending physician in planning for the resident's care. Ensure that care plans indicate occupational therapy when such therapy is ordered. Participate in the development and implementation of care plans, including quarterly reviews. Review complaints and grievances made by the resident, families, and/or visitors and make oral/written reports to the administrator indicating what action(s) were taken to resolve the complaint or grievance. Maintain a written record of the resident's complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint. Allow the resident to participate in the planning and scheduling of his or her treatment. Working Conditions Works in office areas, therapy rooms, resident rooms, etc. Moves intermittently during working hours. Is subject to frequent interruptions. Is involved with physicians, residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
