Duties:
Provides patient care in accordance with established protocols and guidelines.

Assure that patients are treated promptly in accordance with their scheduled appointments.
Convey to patients the importance of complying with established plans of care, including home exercise programs, and minimizing cancellations and no show of appointments
Consults with referring physicians as needed through documentation as well as verbally.
Pursue continuing education in the form of formal classes, reading of periodicals and other literature.
Reviews Physician's referral (prescription) and patient's condition and medical records to determine physical therapy treatment required.
Tests and measures patient strength, motor development, sensory perception, functional capacity, respiratory and circulatory efficiency, and records findings to develop or revise treatment programs.
Plans and prepares a written treatment program based on evaluation of patient data.
Instructs, motivates, and assists patients to perform various physical activities and in use of assistant and supportive devices such as crutches, canes, and prostheses.
Administers treatments, manual therapy, and traction to relieve pain and improve functioning
Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit.
Records treatment, response, and progress in the patient's chart into the computer.
Instructs patients and family in treatment procedures to be continued at home.
Confers with Physicians and other practitioners to obtain additional patient information, suggest revisions in treatment programs, and integrate physical therapy treatment with other aspects of patient's health care.
Requirements:
Master of Physical Therapy (MPT) or Doctor of Physical Therapy (DPT) from an accredited school

Perform routine respiratory care activities, which include aerosol therapy, MDI, DPI, airway clearance, lung expansion therapy, oxygen therapy, non-invasive ventilation, patient education, set up and operate various types of equipment to be utilized in the diagnosis and treatment of respiratory diseases and/or illnesses.
Report any changes in patient's condition or behavior immediately to their assigned preceptor, respiratory campus supervisor or assistant manager.
Request assistance/guidance as needed to ensure that patient care is delivered in a safe, high quality and effective manner at all times.
Responds to patient requests for assistance in respectful, positive and timely manner.
Documents patient information accurately and in a timely manner.
Assumes accountability for all actions in the workplace and reports errors promptly.
Maintains patient confidentiality when faced with questions from visitors or the public.

As a Home Health Speech Language Pathologist, you will:
Evaluate, direct and provide speech/language pathology service to patients in the home or facility
Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
Utilize professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions.
Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician.
Provide instruction and training to patients in use of alternative communication systems when appropriate.
Provide counsel and instruction to patients, families and healthcare staff.
Maintain and submit documentation as required by the Company and/or facility. Prepare and submit timely written reports of evaluations, visits, summaries, care plans, care coordination activities and progress reports as required by Company policy.
Participate in care coordination activities and discharge planning.
Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when the recipient's right to access is clearly established and the sharing of such information is clearly in the best interests of the patient.
Attend, participate in and/or conduct internal staff development programs, obtain continuing education as required by Company policy, regulation.

A valid driver's license, auto insurance, and reliable transportation are required.
