The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.
Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care.
Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.

Job Requirements:
Minimum bachelor's degree in occupational therapy from an accredited school of occupational therapy, successful completion of the National Certification Exam and current licensure from the state where therapy will be provided.
Prefer a minimum of one year's therapeutic experience in a non-home health care environment.
Strong knowledge of theory and concepts relating to the practice of physical therapy and the ability to evaluate and treat patients for whom a physician's order exists.
CPR certified and strong communication and organization skills.
Minimum of twelve hours of continuing education annually.

Summary:
The Clinical Exercise Physiologist works closely with the multidisciplinary healthcare team to coordinate evidence- based exercise therapy that is integrated and compatible with the patient-focused medical goals. Responsibilities:
1. Patient Assessment and Monitoring: Assesses exercise therapy needs using standardized practice and critical thinking. Recognizes potential medical problems in all clinical situations and reports these to MD or RN; Recognizes unsafe exercise situations and intervenes to ensure patient safety; Monitors response to exercise such as heart rate and rhythm, blood pressures, blood glucoses, and O2 Sats. Attends and participates in patient care conferences.
2. Clinical Exercise Prescription: Under the supervision Program Medical Director and/or a CEP II, develops exercise prescriptions according to age, disease, medical history, and individual needs for assigned patients. Implements and monitors exercise prescriptions and individual treatment plans and participates in the discharge planning process. Demonstrates clinical and age-specific competency as evidenced by clinical competency tools, forms, chart audits, tests, observation, etc.
3. Compliance/Clinical Quality Improvement: Assists in achieving compliance with the Joint Commission, Centers for Medicare and Medicaid Services, Healthcare Facilities Accreditation Program (HFAP), American Association for Cardiopulmonary Rehabilitation (AACVPR) or other accrediting agency standards to continuously improve the safety and quality of care to the public and to the organization's performance improvement. 4. Communication: Documents all aspects of exercise therapy in the medical record. What you will do: Responsibilities listed include but not limited to:
Perform evaluations and develop treatment plans
Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
Perform discharge evaluations, make appropriate recommendations for safety, and provide referral and education resources
Communicate patient's needs and progress to the treatment team, physician, person receiving services and family members
Supervision of Occupational Therapy Assistants as needed
Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
Qualifications
What you will need:
Minimum of a bachelor's degree in Occupational Therapy from a college or university with an accredited Occupational Therapy program
Current, unrestricted license as an Occupational Therapist by state in which practicing
Current Live CPR Certification
Two-step TB skin test required
Proof of Auto Insurance
Demonstrates knowledge of evidence based therapy interventions.
Familiarity with assisted living, independent living, geriatrics and/or home health
Communicates effectively and professionally in verbal and written interactions
Ability to lift 50 pounds
Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
Duties require fine motor skills,visual acuity, and walking/ standing for extended periods
Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
A health screen or examination may be required prior to assignment and periodically thereafter, depending on specific location policy, local and state regulations to verify employee is physically capable of performing assigned duties with or without reasonable accommodations

Duties and Responsibilities:
Evaluate new students enrolling in the program within 30 days after entering program if Occupational Therapy needs exist
Use standardized tests, customized measures, and/or observation to evaluate the student in the areas of cognitive perceptual motor skills, motor coordination, psychosocial development, self-care, muscle strength and range of motion, postural reflexes, and needs for adaptive devices and prostheses
Evaluate treatment effectiveness by observation, testing, and communication with family
Utilize treatment procedures involving physical, sensory, manual, creative, and recreational activities
Participate in MET and IEP process
Utilize a team approach to planning and treatment with school personnel, families, physicians, and agencies
Consult with teachers regarding occupational therapy goals for the classroom setting and support classroom goals in the therapy treatment program
Provide training to appropriate individuals regarding effective methods to aid the student in the mainstream
Demonstrate professionalism when dealing with confidential issues and materials
Maintain file of caseload and keep Building Coordinator updated on changes
Design/construct adaptive equipment and devices for the student and/or classroom
Demonstrate willingness to keep updated on issues and research by pursuing professional development
Maintain schedule and log progress toward goals for student's sessions
Promote and participate in specific school projects such as screening programs, career days, etc.
Attend parent/ teacher meetings (As necessary)
Perform other job related duties as assigned by the building principal
