Speech Pathologist (SLP) is a professional member of the home health interdisciplinary team. Provide services that are ordered by the physician as indicated in the plan of care. Develop and evaluate the plan of care in partnership with the patient, representative (if any), and caregiver(s), and in consultation with the physician and other interdisciplinary team members. Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate), including Clinical Supervisor and interdisciplinary team members related to the current plan of care, patient's reaction to treatment and changes in condition. Provide education and counseling based on individualized need for the patient, family and caregiver(s). Evaluate the home environment and make the appropriate recommendations. Instruct other health team personnel, including Home Health Aides and the patient's family, in Speech Therapy Program. Consult and coordinate with other health care disciplines. Participate in Agency-sponsored in-service education, utilization review and team conferences. Provide discharge planning, as appropriate. Evaluate effectiveness and outcomes of care / response to therapy interventions. Assist in developing and updating the plan of care.
