The PT will cover various PACE Physical Therapist duties remotely or in-person for any of the AltaMed PACE Centers, as needed to support compliance-related needs such as assessments, care planning, and IDT/Care Coordination meetings. The PT will support training such as onboarding, process implementation, and competency re-training as assigned by the Rehabilitation Manager. The PT will support Rehab operations by assisting processes such as data collection, data analysis, report building, preparing and facilitating presentations, and compliance recordkeeping. The PT will support PACE by assisting with program planning and implementation, participating in CQI initiatives, and serving on interdisciplinary committees to improve participant health outcomes and the delivery of Rehab services. The PT will collaborate with discipline managers, QI team, the Rehabilitation department, and other PACE leaders to align with organizational goals, provide an exceptional customer experience, and optimize care delivery.
Minimum Requirements
* Physical Therapist license issued by the State of California Physical Therapy Board, coupled with a minimum of 1 year of experience working with the frail elderly (Geriatric) and disabled populations, preferred.
* Current BLS certification is also required.
* Bilingual English/Spanish/Cantonese/Mandarin preferred.

· Maintains patient records.

· Bachelor of Science or Master of Science degree in Physical Therapy.

May provide onsite supervision to Physical Therapist Assistants, Rehab Aides, and provisional physical therapists per state practice guidelines.
