Job Summary:

* All paraeducators must have completed the ETS Praxis, or obtained an AA Degree, or have earned 72 quarter/48 semester credits, or have completed a registered apprenticeship program prior to applying for a paraeducator position in the East Valley School District (documentation required at time of application).

Record treatment, response, and progress in patient's chart and/or automated systems. Instruct patient and family in treatment procedures to be continued at home.

Coordinate treatment with physician and other staff members to obtain additional patient information, suggest revisions in a treatment program, and integrate occupational therapy treatment with other aspects of the patient's health care.

Job Description This licensed, professional position is responsible for physical therapy evaluation, treatment planning and implementation, education and coaching, and discharge planning for patients in accordance with established clinical policies while maintaining the highest degree of quality care. As a member of the patient's health care team, the Physical Therapist assumes responsibility for communication of patient related issues with appropriate team members and/or other clinic staff. Must have an appropriate current state license to practice as a Physical Therapist and maintain current CPR certification.

Demonstrates an ability to treat a wide range of diagnoses and be knowledgeable in current therapy treatments, goals and contraindications for treatment. Seeks consultation, mentorship and/or continuing education when appropriate and to build skills.
Provides education and counseling to patient/family regarding treatments and goals, potential safety risks and/or other issues within scope of practice.
