How you'll contribute
The OccupationalTherapist (OT) provides occupational therapy evaluations, modalities, and treatments as prescribed by a licensed physician in an effort to restore function and prevent disability following injury, disease, or physical disability. Assists patients to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.
Assumes responsibility and accountability as indicated for the care of those patients being treated by Certified OccupationalTherapist Assistants, rehabilitation technicians and occupational therapy students. May be assigned to orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual as qualified to do so.
Assesses patients' needs; identifies and initiates appropriate occupational therapy interventions; works cooperatively with other patient team personnel in maintaining standards for professional occupational therapy practice and interdisciplinary teamwork.
Observes and evaluates treatment affect, accomplishment of short- and long-term goals. Recommends change to physician, if needed.
Performs an appropriate assessment on each patient and performs reassessments, as per policy. Performs accurate and timely QI scoring. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services.
Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Reports patient progress and barriers to discharge in each patient team conference
Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate. Performs appropriate documentation.
Demonstrates knowledge and proficiency of occupational therapy modalities.
Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services.

-Experience with a range of diagnoses including general medicine, orthopedics, neurology, and cardiopulmonary.
-Acute Rehabilitation Unit: Demonstrates competency in Inpatient Rehabilitation Facility Quality Reporting Program (IRF QRP).
-May require travel to patents home for home evaluation.

What You'll Do
Conduct initial and ongoing occupational therapy evaluations in the home
Assess patient functional status, including strength, endurance, coordination, communication, and self-care abilities
Evaluate the home environment for safety risks and barriers to independent living
Develop, implement, and update individualized plans of care in collaboration with the Health Care Provider and RN
Complete initial evaluations, treatment notes, and re-evaluations at least every 30 days
Complete comprehensive assessments and OASIS documentation accurately and on time
Observe, document, and report patient progress and significant changes; obtain updated orders as needed
Identify and recommend adaptive equipment and strategies to improve independence with activities of daily living (ADLs)
Educate patients and caregivers on adaptive techniques and devices
Provide supervision and support to Occupational Therapy Assistants (OTAs), as assigned
Collaborate with physicians and interdisciplinary team members to deliver high-quality care

Qualifications
Bachelor's degree from an accredited Occupational Therapy program
Active OccupationalTherapist license in the state of practice
Minimum of one (1) year of OT experience; home health experience preferred
Valid driver's license, reliable transportation, and auto insurance
Ability to complete home visits and document care accurately and timely
Home health documentation experience preferred (Home Care Home Base a plus)
