Responsibilities
OccupationalTherapist:
Initiates skilled occupational therapy treatments in accordance with physician's orders; maintains required contact with physician.
Maintains written evaluations, progress notes, MDS records and discharge summaries for individuals in occupational therapy program.
Confers with other health care staff regarding special needs of indviduals to enhance rehabilitation or maintenance of function.
Provides input for Care Planning meetings and attends during absences of the Director of Occupational Therapy.
Maintains statistical records of indviduals' treatments.
Assists with in-service training programs upon request.
Records and updates individual's care plan.
Submits requisition for supplies needed in the treatment of individuals to Director of Occupational Therapy.
Assumes all duties of the Director of Occupational Therapy, if applicable, during absences including supervision of COTA's and Occupational Therapy Aides. Assumes responsibility for all necessary documentation.
Qualifications
OccupationalTherapist:
Graduation from an approved school of occupational therapy required.
State license to practice occupational therapy required.
Two years' experience in occupational therapy preferred.
Interest in occupational therapy treatment programs for the geriatric and subacute populations utilizing a team approach.
Ability to direct others necessary.
Ability to communicate well verbally and in writing required.
Current, valid CPR/BLS credential
Sense of responsibility and ability to work well with residents, families, and other staff members required.
Qualities of maturity, diplomacy, and ability to work well with others required.
Ability to work with geriatric and chronically ill residents, as well as other professionals in a friendly and helpful manner necessary.
Ability to perform tasks to established standards of excellence required.

Discusses diagnoses and prognoses with patients and/or caregivers as well as treatment plans and goals of occupational therapy.

Evaluates effects of treatment at various states and adjusts treatments to achieve maximum benefits.
Identify and document goals, anticipated progress and plans for re-evaluation

Fabricate elbow, wrist, hand/finger splints PRN along with recommending prefabricated splints when appropriate

Provides physical therapy services under the supervision of a designated licensed physical therapist. • Lends counsel and education to patients and their family members/care givers (if applicable) regarding the patient's physical therapy services.
• Functions as a clinical instructor for PTA students. (after a minimum of 1 year's work experience).
• Maintains clinical competency by maintaining CEU requirements for NYS licensure.
• Maintains 90% compliance on Chart Audits. Follows required documentation guidelines and expectations set forth by the rehab department and regulatory bodies (Medicare, Medicaid, NYS Ed, DOH, Commerical Insurances, etc.).
• Complies with all agency policies and procedures.
