The Occupational Therapist is responsible for providing in home Occupational Therapy services to Agency patients, as prescribed by the attending physician, and for providing Agency staff, and the attending physician with Occupational Therapy consultation as needed. DUTIES 1. Performs Occupational Therapy assessments of patient functioning levels, plans short and long term goals toward improving/restoring independence and physical/cognitive functioning; and implements developed treatment programs, approved by the attending physician. 2. Provides guidance and instruction to patients/family members of developed therapeutic, creative, and self care procedures/activities; and explains adaptive and progressive approaches in applying it. 3. Instructs the patient/family members in use of appropriate orthotic or prosthetic equipment; and may also design and fabricate customized adaptive and assistive devices, with training for their use. 4. Document findings on the standardized evaluation form timely, accurately and adhering to all guidelines. Provide a comprehensive treatment plan including long and short-term goals, frequency, duration, and treatment modalities. Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges. Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions. Qualifications: Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy that the AOTA recognizes. The Occupational Therapist administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician or advanced practice practitioner's orders and plan of care under the direction and supervision of the Home Health leadership team. In this position you will be required to: Evaluate patient's functional status including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc. Evaluates home environment for hazards and barriers to more independent living Participates in the development of the individualized plan of care including equipment needs. Develops treatment program and establishes goals for improved function Communicates plan of care to referring physician and other organization professionals.
