This role is responsible for assessing, planning, implementing, and evaluating occupational therapy care to help patients regain independence in activities of daily living (ADLs), improve safety in the home, and enhance overall functional ability. The therapist ensures all care is delivered in compliance with Title 22 regulations, Medicare Conditions of Participation, ACHC standards, and agency policies and procedures. Reporting Relationship Director of Patient Care Services/Nurse Manager Scope of Supervision None Responsibilities include the following: Provide skilled occupational therapy services to patients in the home setting, including evaluation, treatment, and patient/caregiver education. Develop, implement, and revise individualized therapy plans of care in collaboration with the interdisciplinary team and the patient's physician. Document all patient visits, assessments, and interventions in accordance with Medicare, Title 22, ACHC, and agency standards for accuracy, timeliness, and completeness. Submit all clinical documentation within 48 hours of each patient visit. Educate patients and caregivers regarding safe techniques, adaptive equipment, energy conservation, and compensatory strategies to maximize independence and safety. Provide training in activities of daily living (ADLs) such as dressing, bathing, meal preparation, and household management as appropriate. Monitor patient progress and communicate changes in condition or plan of care to the physician and interdisciplinary team promptly. Adhere to all agency policies, procedures, and safety protocols while promoting quality patient care and regulatory compliance. Participate in case conferences, staff meetings, and ongoing training as required. Maintain professional knowledge and skills through continuing education and training in accordance with licensure and agency requirements. Minimum Qualifications: Current, active Occupational Therapist (OTR/L) license in the state of California. Minimum of one (1) year recent clinical experience, preferably in home health or acute care. Knowledge of and ability to comply with Title 22, Medicare Conditions of Participation, and ACHC standards. Current CPR certification. Valid California driver's license, reliable transportation, and proof of auto insurance. Strong clinical assessment, communication, and documentation skills. Develops and monitors individualized goals and plans as identified in the Individualized Treatment Plan (ITP) in conjunction with other programs, as needed. ESSENTIAL FUNCTION: Evaluates and treats children referred for occupational therapy services using established evaluation procedure and standardized assessments, particularly in perceptual/sensory motor, fine motor and activities of daily living. Using independent discretion and judgement develops individualized occupational therapy treatment, plans and goals. Partners with interdisciplinary team to provide discipline-specific consultation to other direct-care staff. Develops and provides coaching for family members and/or child to incorporate new skills and/or techniques into normal routines and activities at home. Works with caregivers to implement a home program that achieves occupational therapy goals. Assists in the coordination of services and makes appropriate referrals as needed. Completes necessary documentation including evaluations, treatment notes, progress reports and exit summaries, and keeps updated with current medical reports and other site-related documents on each child. Remains current regarding new research, current trends and developments in autism, special education and related fields. Attends and assists with staff meetings, in-services and other meetings as requested, regarding occupational therapy across program service divisions. Performs other duties as assigned.
