• Provides one -on -one care to youth and young adults when primary care giver is not available.
• Ensure the safety and well -being of the client.

• Aids with daily living, including personal care such as ambulation and personal hygiene assistance. Prepares and maintains records of client progress and services performed, reporting changes in client condition to manager or supervisor.
• Actively promotes client's mental alertness through involvement in activities of interest
• Performs housekeeping duties such as cooking, cleaning, washing clothes and dishes, and running errands. Cares for individuals and families during periods of incapacitation, family disruption, and convalescence; providing companionship, personal care, and help in adjusting to new lifestyles.
• Instructs or advises clients on issues such as household cleanliness, utilities, hygiene, nutrition, or infant care.
• Attends in -service training as required by regulation
• Adheres to all Agency administrative and clinical policies and procedure
• Participates in case reviews, consulting with the team caring for the client, to evaluate the client's needs and plan for continuing services.
Requirements Skills/Abilities:
• Must possess a strong service orientation, including active listening, critical thinking skills, and close monitoring of clients.
• Minimum of three references required
• Must have an automobile and a valid driver's license
• Must have proof of car insurance
• Pass a criminal background check including fingerprinting
Education and Experience:
• High school diploma or equivalent required.
• More than two years or experience preferred.
Physical:
• Prolonged periods of standing, bending and lifting.

Job Functions:
WIC Clerk/PC will be responsible for providing women with basic breastfeeding information, benefits of breastfeeding, overcoming common barriers and handling common breastfeeding issues. Conduct prenatal and postpartum breastfeeding support classes via group or individual contacts, telephone, home visits, and/or hospital visits. Make routine periodic contacts with all clients assigned to their caseload. This position will also be responsible for determining participants' program eligibility, health screenings (blood work and anthropometrics); preparing participant charts, answer phone calls, schedule appointments, filing. Travel within Gateway CAP service area is required. Other duties as assigned by supervisor/program manager.
Education requirements: A High School Diploma or GED and a Valid Driver's license; travel is required. One-year experience with administrative duties and must be computer literate. Have excellent customer service skills; be reliable and able to work with children and be comfortable working in a high paced environment. Ability to work independently, bilingual (Spanish, Creole or Vietnamese speaking) is highly desirable.

Responsibilities and Duties
Monitors the health, safety, and well-being of all residents
Assisting in training and monitoring of medication administration
Supervise care staff in accordance with current state regulations and community policies

Qualifications
Graduation from an accredited School of Nursing, with current LPN license
