Essential Functions:

* Provides direct clinical services in accordance with accepted standards of social/work clinical practice.

An Inpatient Therapist who excels in this role:
Conducts individual and group therapy sessions to educate patients regarding psychological, emotional, or substance abuse problems

Provide compassionate support to families during the organ donation process, addressing their questions and concerns with empathy and understanding.

Act as a liaison between families and medical providers, ensuring effective communication and clarity regarding the donation process.
Offer immediate support and resources to families in crisis, assisting them in navigating complex emotions and providing grief counseling as necessary.
Collaborate with the Family Engagement Manager to develop and refine educational materials, resources, and training programs to enhance family engagement strategies.
Maintain accurate records and documentation of family interactions, ensuring confidentiality and compliance with all regulatory standards.
Participate in community outreach initiatives to promote organ donation awareness and educate the public about LifeGift's mission and the importance of donor families.

Do you have the education and experience to be a Family Engagement Specialist I?
Bachelor's degree in Social Work, Nursing, Psychology, Public Health, Human Services, Communications, Counseling, Education, or a related field; Master's degree preferred.
Minimum of 2-3 years' experience in family support, grief counseling, or a related field, preferably in healthcare or organ donation settings.
Backgrounds that demonstrate advanced interpersonal communication, conflict resolution, and the ability to guide individuals through complex, emotionally charged decisions
Knowledge of the organ donation process and relevant regulatory frameworks is highly desirable.
Ability to navigate sensitive and complex situations with professionalism and poise.
Proficient in Microsoft Office Suite and experience with data management systems.
Bilingual (English/Spanish) is a plus.

Target population is high acuity patients with mental health, homelessness and/or substance use issues. Additionally, there will be cross-training to cover the Integrated Navigation model which performs teleassessments for bed placement purposes and assist with escalated mental health crisis calls, coordination with CHCS liaisons, community entities, and linkage to CHCS services.
Focus efforts include but not limited to:
* Identifying consumers currently being treated by CHCS and provide connection back to assigned CHCS case manager to ensure their mental healthcare needs are being met.
* De-escalating on scene of a client in crisis by providing crisis intervention to individuals to reduce symptoms, provide education and prevent admission to a more restrictive environment.
* Assisting clients with accessing appropriate social services resources (i.e. housing, clothing, identification documentation (IDs), food, water, hygiene materials).
* Reducing the utilization of Emergency Detention Orders (EDOs) and assess for least restrictive environment.

Crisis Response Clinicians (CRC) work in collaboration with public safety agencies to include law enforcement and Emergency Medical Services (EMS) under the guidance of the Southwest Texas Regional Advisory Council (STRAC). A collaborative effort with First Responders - San Antonio Police Department Mental Health, Bexar County Sheriff Office Mental Health, South Texas Advisory Council (STRAC), Department of Human Services (DHS), EMS Providers - San Antonio Fire Department Mobile Integrated Health Team (MIH) and Acadian Ambulance aimed at addressing 9-1-1 mental health related calls more effectively by responding with a trained multidisciplinary team to divert individuals experiencing a mental health crisis away from jail and emergency departments and instead to the least restrictive environment. Target population is high acuity patients with mental health, homelessness and/or substance use issues. Additionally, there will be cross-training to cover the Integrated Navigation model which performs teleassessments for bed placement purposes and assist with escalated mental health crisis calls, coordination with CHCS liaisons, community entities, and linkage to CHCS services.

JOB DESCRIPTION
GENERAL DESCRIPTION:
The mission of Metrocare Services is to serve our neighbors with developmental or mental health challenges by helping them find lives that are meaningful and satisfying. We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve. The Metrocare Services Mental Health and Primary Care Division includes a Crisis Services Program that provides 24/7 services to those in need via the crisis hotline and mobile crisis outreach teams. Crisis services are offered for to anyone, anywhere, and at any time the need arises. Crisis services will be provided regardless of an individuals ability to pay or whether he/she is homeless or unable to produce verification of residence. The Crisis Interventionist Specialist-QMHP plays a vital role on a team comprised of a Licensed Practitioner of the Healing Arts and a Mental Health Peer Specialist.
Work Hours: 11am-7pm (2nd Shift)
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The essential functions listed here are representative of those that must be met to successfully perform the job.
Provide crisis intervention services in a variety of modalities, including telephonic, telehealth, and face to face intervention.
Ensure follow up is provided within 1 hour of the emergent crisis
Assists inpatient facilities, including emergency departments, behavioral health hospitals, state hospitals, residential treatment facilities and the criminal justice system, in continuity of care arrangements for consumer referred to Metrocare
Hotline staff screen/assess the crisis, provide an evidence-based intervention, and stabilize the crisis. The caller is then linked with service coordination assess the level of further stabilization needs and appropriateness for other services in the crisis continuum
When Hotline interventions are not able to adequately stabilize the situation, the MCOT team to further assess the crisis. If screening and assessment indicates that there is an emergent/imminent risk of harm to self or others, the Hotline staff notifies 911 and attempts to stay on-line with the caller until 911 personnel arrive on site where the individual is present.
Develop person-centered crisis plan and assist the individual in executing the plan during and post crisis intervention.
Ensure continuity of care post crisis intervention to appropriate outpatient services, including but not limited to, outpatient mental health, substance abuse services, and/or primary care services.
Be accessible for referrals and communication through our electronic health applications, by phone and email to provide a timely and comprehensive response.
Interacting with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services
Report all clinical crisis staffing to supervisor or designee
Facilitate communication between providers within a schedule that meets outlined timeframes
Develop close working relationships with internal and external referral resources
Maintain confidentiality of information concerning individuals and family members
Attends and participates in all scheduled team meetings.
