MAJOR FUNCTIONS: An Office of Mental Health (OMH) Housing Case Manager plays a crucial role in assisting individuals with mental health challenges to secure and maintain stable housing. The primary responsibilities include conducting assessments, developing personalized service plans, connecting clients with necessary services, and ensuring compliance with housing regulations. The role requires a combination of direct client support, coordination with service providers, and administrative tasks to promote clients' independence and well-being. SPECIFIC DUTIES & RESPONSIBILITIES: Client Assessment and Service Planning: Conduct comprehensive assessments to identify clients' housing and support needs. Develop individualized service plans with short-term and long-term goals, regularly reviewing and updating them to ensure they meet clients' evolving needs. Service Coordination: Connect clients to essential services, including medical care, mental health support, substance use treatment, legal assistance, and entitlement programs. Act as a liaison between clients and external organizations to advocate for their needs. Direct Support and Monitoring: Provide ongoing support through regular home visits (at least one per month) and telephone contacts. Monitor clients' progress toward their goals, document interactions, and adjust service plans as necessary. Crisis Intervention: Respond to crisis situations promptly, employing de-escalation techniques and coordinating with appropriate services to ensure clients' safety and stability. Documentation and Compliance: Maintain accurate and up-to-date case records, including service plans, progress notes, and other required documentation. Ensure all activities comply with OMH guidelines and regulations, preparing for audits as necessary. Client Empowerment: Assist clients in developing life skills necessary for independent living, such as budgeting, cooking, and community navigation. Provide education on tenants' rights and responsibilities to promote self-sufficiency REQUIREMENTS: EDUCATION AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES: A bachelor's degree in social work, psychology, or a related field is typically required. At least one to two years of experience in case management or working with individuals with mental health disorders. Experience in housing assistance and knowledge of OMH programs are preferred. They work directly with the clients in individual and group counseling sessions on issues surrounding chemical abuse/dependency and advocates a drug-free lifestyle. The Case Manager works closely with the rest of the staff as an integral part of the clinical team. SPECIFIC DUTIES & RESPONSIBILITIES: Provide individual counseling and case management services addressing all medical, psychiatric, housing and vocational concerns. In collaboration with each client, formulate treatment plan, monitor achievement of objectives, review and update treatment plan in accordance with regulatory requirements and as clinically indicated. Maintain up-to-date client clinical records including, at minimum, treatment plans, group counseling notes and progress notes in accordance with regulatory guidelines and program standards. Conduct education/prevention groups to address topics which address chemical dependency and related issues. Provide crisis intervention services. Responsible for ongoing client discharge planning. Perform room inspections and addresses client's needs in relation to successful completion of Activities of Daily Living (ADL), including money management, medication management, and personal hygiene and other life skills which support recovery. Provide other therapeutic services through active participation, observation and evaluation of each assigned clients engagement in the resident hierarchy, job functions, and social and recreational activities. Qualifications for Position: 1) Must have lived experience with substance use, recovery, and/or harm reduction. 2) Certified Recovery Peer Advocate (CRPA) credential is required or must be obtained within one year of hire. 3) Previous experience in street outreach, harm reduction services, or peer navigation is strongly preferred. Skills and Knowledge Required: Strong skills in trauma-informed care, harm reduction practices, overdose prevention, and motivational interviewing are essential, along with the ability to engage clients with empathy and respect. The Outpatient Counselor documents all provided services in a manner that is compliant with regulatory and third-party payor requirements. She/he works to coordinate and provide treatment services based on the individualized treatment plan including discharge planning. The Outpatient Counselor advocates for and promotes a recovery-based lifestyle. The Outpatient Counselor works closely with the rest of the staff as an integral part of the clinical team. SPECIFIC DUTIES & RESPONSIBILITIES: Administer comprehensive psychosocial assessment tools at intake and at various points in accordance with Odyssey House policy and procedures. Provide individual counseling, group counselling and case management services onsite and in the home or community. In collaboration with each client, formulates a comprehensive individualized treatment plan, monitors achievement of objectives, reviews and updates the treatment plan in accordance with clinical needs of the client. Maintain up-to-date client clinical records including at minimum: treatment planning, group counseling notes, progress notes, and the monitoring of social determinants of health in accordance with regulatory guidelines and program standards. Provide crisis intervention services. Responsible for ongoing client discharge planning beginning at admission. Provides ongoing assessment of each assigned client, monitoring for any changes in clinical needs. Assures timely an effective care coordination with collateral services providers, family members and criminal/family justice agencies. Delivers clinical services in accordance with evidence-based curricula and training. Acts as a liaison with all referral agencies providing regular verbal and written progress reports. Establish community and agency affiliations which address the client's need and strengthen the delivery of services. Provide education and direct services to family members, as appropriate, regarding client's substance use/misuse and related behavioral issues. Conduct collection of samples for toxicology randomly and as needed. Attend regularly scheduled staff meetings and case conferences. Attend all required in-service training seminars. Participate in quality improvement activities. Other relevant duties as required. REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES CASAC or CASAC-T preferred, or the ability to obtain within 6 months of hire. Ability to deliver culturally competent services to a diverse client population in an urban setting. Proficiency with computer operation (Microsoft Word, Excel, Electronic Health Records, and Outlook programs). Must be able to work a flexible schedule.
