The Chaplain Essential Duties Are:
Offers care grounded in initiating, developing, deepening and closing a spiritual and empathic relationship with those receiving care.
Qualifications:
Master of Divinity degree (M. DIV) or equivalent in Theology, preferred.
Preferred candidates are ordained or commissioned ministers that are ecclesiastically endorsed by their recognized faith group or religious organization.
Preferred candidates should be board certified, eligible for certification, or working towards professional certification by APC, BCC-HPCC, NACC, or NAJC.
Clinical Pastoral Education (C.P.E.), 4 units preferred, minimum 1 unit and enrolled for completion.
Three years of current active pastoral ministry or chaplaincy experience.
Computer skills sufficient to properly document services and care.
Ability to drive during daytime, nighttime or inclement weather.
Valid Driver's License with Safe Driving Record

The Chaplain provides spiritual support, guidance, and resources to patients and their families. They provide compassionate services to a diverse population encompassing all faiths that are represented by patients, families, and team members, while at the same time, reflecting the mission and philosophy of Suncrest.
Qualifications
Certificate of graduation from an accredited seminary or school of theology (BD, MDiv or equivalent theological degree), or a degree in pastoral care, or equivalent experience; Master's degree preferred
One year of chaplain experience preferred
Experience working with death and the dying, as it relates to patients and their families/caregivers;
Ability to work as a contributing member of interdisciplinary group
Comfort level in inter-faith setting
Minimum of 1 unit of CPE, preferred; and MDiv or Masters in Theology required.
Assesses spiritual needs of patients and families/caregivers in the hospice program and ensures that appropriate services are rendered in a timely manner;
Serves as liaison and support to community chaplains and spiritual counselors;
Maintains accurate records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision;
Attends patient care conferences and interdisciplinary group meetings, providing consultation, education, and support
Provides for funeral or memorial services for patients as requested;
Assists with the supervision of spiritual care volunteers when assigned to patients/families/caregivers;
Actively participates in quality assessment performance improvement teams and activities.

Department Description: Wilson Health's Home Health & Hospice Care is designed to provide comprehensive healthcare to patients in their own home on a temporary basis. The care is provided under the direction of a physician, and the goal is to help patients regain their independence after an illness, injury or hospitalization. The goal for patients with chronic illnesses may be to keep them at home safely and as pain-free as possible. Services provided are Skilled Nursing Care, Home Health Aide Care, Social Worker Care, Physical Therapy Care, Occupational Therapy Care, and Speech Therapy Care.
Job Summary: The Home Health Care & Hospice Chaplain is supervises activities related to the spiritual, relational, and/or emotional needs of the Hospice patient/family.
Essential Duties & Responsibilities:
* Provides assessment reflecting the spiritual and relational aspects of the patient and or caregivers situation through the spiritual assessment and visit.
* Prepares a spiritual plan to address specific issues identified by the spiritual assessments. Shares plan with appropriate staff members.
* Is an active member of the interdisciplinary team and participates in team conferences every two weeks.
* Communicates impressions of ongoing visits and assessments with other staff involved in the case management as well as with outside clergy involved. Offers assistance through participation in Hospice team conference.
* Through work with the client/family assists them to constructively deal with the specific issues that are being addressed refers them to other resources as needed.
* Completes required documentation on a timely basis.
* Chaplain provides bereavement services including phone calls, visits and memorial services to family members upon the death of a patient.
* Must be knowledgeable of and comply with Medicare Hospice conditions of Participation-Spiritual Caregiver.
* Must be able to travel with in a 6 county radius

As a Hospice Chaplain, you will responsible for the provision of spiritual support to patients and families in keeping their belief system. Providing direct spiritual support to patients and families
Working with team members, clergy, and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss
Participating in patient conferences/meetings to explore and assess the spiritual needs of patients/families
Providing bereavement follow-up services as indicated by the plan of care
Developing and maintaining a resource group of clergies to whom specific aspects of spiritual care may be delegated
Preparing accurate patient documentation and submitting
