Reviews and updates plan of care including interventions, medication profile, and visit frequency to meet the patient's changing needs. Assess and reports to physician changes of patient condition, level of care needs, and facilitate determining eligibility for services. Collaborates and participates as member of hospice team in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Team (IDT) meetings, to ensure coordination of services.
