Participates in Continuous Quality Improvement activities to support the quality of Pure Healthcare services.     What You Should Know About the LPN Team Float Nurse Role: This is a DRIVING position and will travel to see patient in both homes and facilities within the Middletown OH (Butler/Warren Counties) and surrounding areas This is a FT position, M-F varied hours of 8:00A-5:00P We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed The LPN Team Float Nurse Essential Duties: The LPN Team Float Nurse provides quality nursing care to patients in a manner that meets the patient and family needs working under the direction and supervision of an RN Care Manager. Qualifications: LPN's -Registered LPN with the State of Ohio without any board actions. One year of previous nurse experience. CPR Certified Certification in Hospice & Palliative Medicine (CHLPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.) Computer skills sufficient to properly document services and care. Ability to drive during daytime, nighttime or inclement weather.
