    What You Should Know About the RN Admission Care Liaison Role: The Admission Care Liaison is a DRIVING position supporting Ohio's Hospice South Region which cover the greater Miami Valley and surrounding areas Schedule hours are day/evening shift 10am-10:30pm with every3rd weekend rotation and 1-2 holidays per year We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed The RN Admission Care Liaison Essential Duties Are: Ohio's Hospice patients will receive quality nursing care and coordination of hospice services, when the RN/LPN Admissions Care Liaison admits the patient into the hospice program efficiently and provides the patient and family with the appropriate resources needed at the end of life. The RN/LPN Admissions Care Liaison will maintain a conversion rate of 75% and skillful use of the electronic medical record by accurate documentation at the point of care. RN Admissions Care Liaison Qualifications: RN's - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years acute care nursing experience preferred. CPR Certified Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.) Computer skills sufficient to properly document services and care. Ability to drive during daytime, nighttime or inclement weather. Valid Driver's license with Safe Driving Record. State minimum automobile insurance coverage.
