Position Summary:
The Clinical Diversion Specialist will ensure the appropriate placement of youth throughout our array of referral and treatment services for the Cuyahoga County Juvenile Court EIDC (Early Intervention and Diversion Center) and CALM (Coordinated Approach to Low level Misdemeanor) Program. He or she will provide clinical assessments based on verbal, written, and face-to-face interviews to determine appropriate level of treatment for youth either referred to the Intervention Center or brought to the program by the police for domestic violence calls. The Clinical Diversion Specialist may explore alternatives such as kinship placement or respite placement for up to three days, in addition to respite if deemed necessary to remove youth from the home. In less acute cases, he or she will connect youth to appropriate mental health and prosocial services. When necessary, the Clinical Diversion Specialist will complete a youth screener, a safety plan, symptomology screeners, and connect the youth and guardian with ongoing services. This program provides services 24/7.
PRN on-call hours are Saturday and/or Sunday from 8am-12a. PRN staff answer these calls remotely and only go into the office if there is a respite placement. All other duties can be completed remotely.
Responsibilities Include:
Responsible for completion of behavioral health and juvenile justice screeners.
Communicate results of assessment and recommended services to referring court/law enforcement staff.
Administer, track, and submit outcome measures in accordance with program standards.
Responsible for case management and referral tracking and ensuring proper funding of all cases before the case leaves intake.
Face-to-face or virtual admission with clients and guardian including, but not limited to, completing all required clinical and administrative paperwork, completing and submitting behavioral health reports and safety plans to referring court personnel, and, when applicable, completing Psychiatric Diagnostic Evaluations and Treatment Plans for new admissions to Applewood Centers' programs.
Coordinate and link with other community behavioral health and substance use providers to ensure continuity of care for open cases.

JOB SUMMARY
The Licensed Crisis Therapist Intern will provide Mobile Response and Stabilization Services (MRSS) to youth (age 2-20) and families who have called the crisis hotline with a family identified crisis. The Licensed Crisis Therapist Intern will respond face-to-face (in person or via video conferencing) with another MRSS Team member to high-risk crisis situations within an hour to assess the youth and stabilize the family. After the initial 72 hours, the family will receive ongoing stabilization and care coordination services from the MRSS team.
ESSENTIAL DUTIES
Provide mobile response to a family identified crisis within one hour.
Engage youth and families in de-escalating the crisis and assess for risk.
Facilitate hospitalization for clients as necessary.
Complete a Crisis Assessment and CANS to help develop a safety plan and an MRSS plan.
Complete 72 hour follow up with clients in collaboration with QMHS staff.
Carry a small amount of clients through the ongoing stabilization phase of up to six weeks.
Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
Respect the privacy of clients and hold in confidence all information obtained during the client's treatment according to HIPPA guidelines. All client-related documents should be handled in accordance with Agency guidelines on confidential material.

Under the administrative/clinical supervision of the School Based Supervisor/ Field Instructor, the School Based Services Intern provides assessment, individual and group counseling, consultation and prevention services to clients, their families, school staff and significant others as needed. Services are delivered to students in a variety of ways, and services can be bundled together to meet the specific needs of the students, faculty and/or families.
INTERN EXPERIENCE DETAILS
School Based Services Interns provide the following services, including, but not limited to:
Individual, family and/or group counseling
School and family education
Case Management
Individual skill trainings
Advocacy
Information and referral
Group work
Crisis intervention
Assessment
Other services necessary to the enrichment of the internship environment.
Location: This internship experience will be based in Southwest Lorain County primarily in Oberlin, OH.
Hours: Field hours are flexible. Occasional evenings should be expected.
Qualifications: This internship is for second year Masters' level students. Therefore, a Bachelor's Degree is required and you must be currently enrolled in a related Master's program. A SWT, CT, LPC, MFT or LSW license is also required.

Work Schedule:
* Start and End times for the role may vary. A typical day may begin anywhere from 8a - 11am and end between 4 - 7pm
* The ideal candidate must be willing to be flexible and willing to adjust their hours to meet community needs
* Training Schedule will be mostly in office during the first 6 months of hire

Essential Functions:
* Demonstrate the values and guiding principles of Our Promise each and every day, performing with the highest standards of excellence, integrity and ethics.
* Provide intensive or moderate care coordination services to assigned clients (children and young adults (up to age 21) with severe and persistent mental health issues).
* Develop and maintain a service relationship with assigned clients, family, caregivers, child, and family team members to provide them support as appropriate.
* Provide structured service planning and care coordination using High-Fidelity Wraparound practices; develop and monitor the child and family-centered care plan to ensure that services are delivered in accordance with plan.
* Serve as a liaison to coordinate, makes referrals, or arrange for appropriate community resources when needed, acts as an advocate for the client and family as needed.
* Complete all assigned training including training regarding cultural and trauma-informed care competency within three months of hire and annually thereafter.
* Accurately and promptly, document and maintain the confidentiality of client and family interactions including, legal, financial, and clinical documentation within agency guidelines.
* Identify urgent, unsafe and crisis situations and escalate to supervisor.
* Attend and participate in agency/ team/clinical meetings, including mandatory trainings, and treatment coordination.
* Maintain ongoing frequent contact with other assigned OhioGuidestone professionals, parents or outside agency professionals assigned to case.
* Facilitate discharge planning and/or transition activities as required.
* Promote an inclusive work environment and present the Agency in the most positive light with all internal and external contacts.
* Exhibit positivity, flexibility, and a willingness to take on new responsibilities as requested or required.
Education/Experience:
Must have an appropriate education/experience:
* High school diploma or equivalent and 3+ years experience managing complex cases and the ability to navigate state and local child-serving systems with knowledge of available community resources to facilitate client care; or
* Associates or Bachelors Degree and 2+ years experience managing complex cases and the ability to navigate state and local child-serving systems with knowledge of available community resources to facilitate client care; or
* Masters Degree and 1+ year experience managing complex cases and the ability to navigate state and local child-serving systems with knowledge of available community resources to facilitate client care.
* Demonstrated ability to work with children and young adults (through age 19) with severe and persistent mental health issues.
* Ability to demonstrate competency in behavioral health skills, for example:
* understanding of human behavior, mental illness, and impact on functioning and behavior;
* understanding how to engage a mentally ill person, concepts of recovery/resiliency
* understanding of the mental health system; de-escalation techniques
Qualifications:
