Job Information Exemption Status: Non-Exempt The Clinical Staff Pharmacist provides comprehensive pharmaceutical care to patients by optimizing medication therapy, providing disease state prevention and management, and promoting health and wellness. Education Qualifications

What you will do
Review, interpret and verify provider medication orders making any recommendations and clinical interventions as necessary based on patient specific condition, disease state, age and other clinical factors.
Manage medication use including the compounding, preparation, selection, dispensing, storage and stocking of medications for the entire organization.
Facilitate completion of pharmacy medication distribution functions. Examples include directing pharmacy technician workflow and verification of accuracy of all final products prior to dispensing to patients.
Address and complete provider consults proficiently and provide drug information to healthcare professionals and education to patients.
Educate and train pharmacy personnel and other healthcare professionals and serve as preceptors for pharmacy students and pharmacy residents.

I. JOB SUMMARY
The Ambulatory Care Pharmacist is responsible for provision of comprehensive outpatient pharmacy services in LMH Health ambulatory care clinics. The pharmacist works collaboratively with prescribers under collaborative practice agreement to facilitate optimal patient outcomes regarding safe medication use. The pharmacist also cross-covers in the LMH Health System Pharmacy for a staffing component.
ESSENTIAL JOB RESPONSIBILITIES
Meet with patient in person or via telemedicine to assist in chronic disease state management
Collaborate directly with providers and through collaborative practice agreements to facilitate chronic disease state management
Serve as a drug information resource for providers regarding appropriate mediation use in the outpatient setting
Vaccinate patients under immunization protocol
Actively engage in professional development to maintain current skills and knowledge of clinical standards of care in the outpatient setting
Participate in organizational and departmental performance improvement efforts to strengthen existing systems, processes, policies and procedures by introducing innovative ideas that lead to improved pharmaceutical care in clinical areas and in the pharmacy department as a whole.
Share pharmaceutical expertise by coordinating and providing educational presentations or in-services for pharmacy and non-pharmacy staff.
Support pharmacy residency program, clerkship students and interns by precepting organized rotations, providing timely and constructive feedback, participating in training and orientation, providing mentorship, and collaborating on presentations and projects.

Job Summary

Supervises Pharmacy Technicians to ensure the compliant and efficient operations of the pharmacy. Checks all work of Pharmacy Technicians in line with State Boards of Pharmacy regulations and provides guidance on complex prescriptions, pharmacy automation systems, orders, and other issues.
Adjusts dosages based on established protocols for each drug, ensuring that dosages are appropriate for the patient's condition and in light of other medications the patient is using and completes clinical tasks related to antimicrobial stewardship.
Educates patients on the proper use and expected effects of medications, especially when patients are using multiple medications, are beginning new regiments, or are being discharged.
Manages narcotics and inventory control and compliance, including regular documentation and completion of checklists. Audits outdated medication in states where Pharmacy Technicians are not permitted to do so.
Contacts Nurses and Physicians as necessary to verify orders and obtain additional information regarding the order.
Assists the Pharmacy Director in maintaining the pharmacy's accreditation and in implementing operational changes within the pharmacy, such as the use of new systems or adjustments of standard procedures.

Incumbents are also accountable for: maintaining a clean and safe environment; using appropriate aseptic technique; handling controlled substances appropriately; completing all educational modules; attending regular staff meetings; keeping abreast of departmental changes/current CSH policy and procedures and The Joint Commission requirements.

In addition to bringing your whole self to the workplace each day, qualified Pharmacy Techs will bring the following experience:
Required Education and Experience
High School Diploma/G.E.D. equivalent
6 months compounding of IV medication
Required Licensure and Certifications
Registered Pharmacy Tech in the state in which the employee works
Active CPT-EXCPT or CPT-PTCB or CPHT certification in good standing in the state in which the employee works as required by the state
Required Minimum Knowledge, Skills, Abilities and Training
Knowledge of the standards, techniques and tools relating to pharmaceutical patient care.
Knowledge of the regulatory requirements applicable to pharmaceutical patient care.
Knowledge of mathematical functions, including addition, subtraction, multiplication, basic algebra and calculations.
Ability to mix oral powders and compounded drugs and to prepare intravenous admixture and other parenteral drugs.
Ability to understand and use/apply I.V. admixture, automated distribution systems, outpatient services and decentralized pharmacy services. This may include hazardous compounding requirements where appropriate.
Ability to understand and apply standards, procedures and techniques to the work assignment.
Ability to read, understand and communicate in English sufficient to perform the duties of the position.
Ability to use office equipment and automated systems/applications/software at an acceptable level.
Ability to establish and maintain effective working relationships as required by the duties of the position
