JOB SUMMARY/OVERVIEW: Enhanced Care Management (ECM) is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of eligible members with complex medical and social needs. ECM uses a comprehensive care management model that is community based, interdisciplinary, high touch and person centered. The Behavioral Health Case Manager ECM: provides case management and coordination of services to target populations. The case manager is a part of a collaborative team working to provide stability and resources to individuals living with a variety of complex physical and mental health circumstances. The case manager facilitates communication between the patient and their care team across departments and outside agencies.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING
Education:
High school graduate or equivalent required.
Bachelor's degree in related field preferred.
Bachelors in social work or another related field preferred.
Experience/Lived Experience:

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
1.
Provide care coordination to clients with multiple medical needs and/or those determined to be at high risk for complication
2.
Provides consistent encouragement, hope, and non-judgmental support to patients with various challenging circumstances.
3.
Motivates ECM Members to be active and engaged participants in their health and overall wellbeing.
4.
Maintains a caseload of approximately 35-40 ECM Members
5.
Maintains updated ECM Member status and uploads Releases of Information, and Care Plans onto Point Click Care.
6.
Develops a care plan and works with the ECM Member to formulate and execute realistic, time specific and measurable goals.
7.
Comply with all Enhanced Care Management operating procedures and reporting requirements.
8.
Assists ECM Members in utilizing community services, including scheduling appointments with social services agencies, and assisting with completion of applications for programs for which they may be eligible.
9.
Identifies and addresses social determinants of health. Develops relationships with local community organizations to implement interventions that address social determinants of health
10.
Experience working successfully with issues of substance use, mental health, criminal background, and other potential barriers to economic self-sufficiency.
11.
Accompanies ECM Members to medical and other service-related appointments when necessary.
12.
Maintain clerical duties: medical record documentation, patient tracking forms and data collection.
13.
Work with all members of the care team and other departmental staff to expedite proactive solutions to issues, serving as primary patient contact for issues concerning care access and service.
14.
Participates in staff meetings. Assists with coordination of care offering input on patient progress.
15.
Collaborates with ECM team and CommuniCare+OLE staff including nursing, medical, dental, behavioral health, and care coordination to ensure the full spectrum of ECM services and benefits are implemented

JOB SUMMARY/OVERVIEW: CommuniCare+OLE is a Federally Qualified Health Center providing comprehensive, patient-centered perinatal, primary care services including medical, dental, substance abuse treatment, behavioral health, and outreach services to low income, multicultural, uninsured, and underinsured patients. The midwife works as part of a multidisciplinary team that includes health educators, nutritionists, lactation consultants, social workers, midwives, physicians, nurses, and other staff. The Midwife also works in collaboration with Sutter Medical Group OB/GYNs, and Sutter Davis Hospital to provide full scope midwifery services. Services are provided at three CommuniCare+OLE clinic sites in Yolo County and at Sutter Davis Hospital.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING
Education:
Completion of an accredited Certified Nurse Midwife program.
Board certification by American College of Nurse Midwives.
Experience/Lived Experience:
1-2 years minimum experience providing full scope midwifery care required.
Experience working in a clinic or FQHC serving a low-income population preferred.
Special Skills/Training:
Bilingual Spanish preferred.
Ability to operate all equipment used in clinic sites and in hospital practice.
Ability to perform all procedures listed in outpatient and inpatient Perinatal Protocols.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
1.
Perform all aspects of routine antepartum, intrapartum, and postpartum midwifery care, as well as preventive health services and comprehensive birth control services, as specified in written protocols approved by the Associate Director of Perinatal Health Services and Medical Director. This includes health history, physical examination, diagnosis, treatment, and health education.
2.
Provide intrapartum care at Sutter Davis Hospital as specified in written protocols and practice agreements approved by Collaborative Practice midwifery/obstetric team. Independently manages and delivers low-risk patients; assumes co-management of high-risk women when appropriate; delivers patients followed by a physician when OB on call is in surgery; informs patient and their family of progress and involves them in any treatment, decision or plan; and manages all patients in Labor and Delivery when OB on call is in surgery; manages complications and emergencies independently until physician arrives.
3.
Order and interpret lab findings for outpatient and inpatient care and consult or refer as needed as well as communicating with patient in a timely manner.
4.
Complete CommuniCare+OLE and Sutter Davis Hospital notes within appropriate timelines according to policies.
5.
Request appropriate consultation with collaborating physicians as needed or specified by protocol.
6.
Communicate follow-up information and high-risk conditions to the Collaborative Practice team during consults with collaborating physicians and midwives and at weekly MD Review/High Risk meetings.
7.
Refer appropriately to team members in keeping with the concept of multi-disciplinary team-based care.
8.
Communicate on-call schedule changes and requests for time off with adequate notice to the Perinatal Program Manager and Associate Director of Perinatal Health Services.
9.
Coordinate with the Volunteer and Learner Program Supervisor to meet volunteer needs of the program. Engage volunteers and/or students/residents to meet program needs; may include duties such as interviewing, selecting, supervising, training, and managing performance. Provide direct and timely performance and behavior feedback as necessary for volunteers and/or students/residents.

Participate in CommuniCare+OLE and Sutter Davis Hospital Birth Center quality improvement and department meetings.

Responsible for providing on-site and community-based case management, care coordination, service linkage and services for those persons seeking support.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING
Education:
Bachelor's Degree in related field preferred or experience in lieu of degree.
Experience/Lived Experience:
Experience working in a mental health, social service, or field-based/community setting required.
History of working with people experiencing substance use disorders and/or homelessness and chronic poverty strongly preferred.
Special Skills/Training:
Bilingual - Spanish/English strongly preferred.
Computer skills to include Microsoft Office (Word, Excel, Outlook).
BLS Certification Required.
Drivers License Required.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
1.
Actively support, encourage, and link individuals seeking to access mental health and substance use disorder treatment services, benefits, or other stabilizing supports by continual efforts to be responsive, flexible, client-centered, and non-judgmental.
2.
Serve as a primary in-person case manager, co-creates case plans with clients, and actively consults with managers and supervisors when situations arise.
3.
Collaborate effectively with internal and external partners in order to support the goals of each client.
4.
Presents options to clients with cultural humility and respect for personal choice.
5.
Demonstrate competence with the technology required to perform job duties including Language Line, Microsoft Office, Avatar, Epic and video-based telehealth and meeting platforms utilized by the agency.
6.
Complete documentation within 48-hours of services in accordance with working hours, all documentation must comply with Medi-Cal, payor, and/or contractual guidelines.
7.
Respond to calls, emails, and other correspondence from internal and external partners in a timely and professional manner.
8.
Monitor charts to ensure the timely placement of all correspondence, completes all screening and data collection tools according to protocol and best practices. Supports with data collection and reporting.
9.
Transport clients in agency vehicles as safe to do so.
10.
Maintain confidentiality and the privacy of protected health information unless permission is provided, or a situation warrants disclosure (e.g. mandated report, safety).
11.
Comply with all legal, ethical, and safety expectations, including requirements related to annual training plans and incident reporting.
12.
Is prepared for and actively participates in staff and program meetings, tracks decisions and information, and makes positive contributions.
13.
Represents self and agency in alignment with CommuniCare+OLE mission and values during all internal and external interactions.
14.
Maintain regular working hours and maintain an accurate calendar reflective of tasks and services.
15.
Performs other duties as assigned.
