We are adding full-time and part-time Drug and Alcohol Technician positions to support various employers who require pre-employment, reasonable suspicion, post-accident, and random drug and breathe alcohol screening. This position will require the collection of bodily fluids and specimens such as urine, hair, and/or saliva. It may also require observing donors providing specimens. This position is responsible for completing drug and alcohol collections in accordance with the Department of Transportation (DOT) and Non-DOT industry standards, as well as evidential breath alcohol testing requirements. The ideal candidate will have the ability to work independently with minimal supervision; ensure protection and confidentiality of personal and healthcare data collected; provide exceptional customer service with a positive attitude and demeanor to all employers and employees being tested; and maintain, at least, a 98% Quality Assurance (QA) rate for all tests performed. The ideal candidate will have the flexibility to work at various locations for scheduled and unscheduled testing to meet the needs of the employers. Candidates will be required to respond to requests for unscheduled services (post-accident and reasonable suspicion testing) within 1 hour of receiving requests.     TITLE Medical Case Manager The Medical Case Manager assists individuals living with HIV in accessing and adhering to health care services linking them with core clinical, psychosocial and other related services. MINIMUM QUALIFICATIONS Bachelors Degree in social services, public health, health administration, human services field or an Associates degree with appropriate certifications (i.e., CADC, LPN) with at least three (3) years experience in social services (case management experience preferred). Must be knowledgeable about HIV/AIDS. Experience serving homeless, ex-offenders, individuals living with HIV/AIDS and/or substance abuse population is preferred. Must have computer literacy and data entry skills. Strong organization and time management skills required as well as the ability to work independently. RESPONSIBILITIES * Manage an assigned caseload of client with HIV/AIDS requiring comprehensive Ryan White Medical Case Management Services. * Conducts initial intake and ongoing assessments, medication and treatment adherence assessments, treatment/service plans and medical assessments * Monitors progress toward improving/stabilizing health status * Development of comprehensive care plans with clients that integrates medication and treatment adherence into services * Work in conjunction with clinical providers (e.g. case conferencing, huddles); ensuring all clients are case conference at least twice per year. * Provides and tracks referrals and linkages to services with special emphasis on core clinical services, access to medical, income benefit programs and access to emergency financial assistance. * Monitors treatment and medication adherence for all core clinical services * Maintains client file, records all activity, and maintains appropriate documentation of eligibility * Inputs all data into the client level database. * Completes the Case Management Competencies training with a passing grade as determined by the AIDS Foundation of Chicago training policy * Attends at least 12 trainings during the contract year to enhance skills and HIV knowledge; including at least four clinical training * Submits monthly/quarterly/annually reports on program/client activities in a timely fashion. * Attends monthly team meetings and all other relevant program meetings as scheduled. * Conducts presentations and/or educational workshops on behalf of CCHC. * Conducts peer chart reviews to ensure continued quality of services. * Completes at least one telephone contact with each client monthly. * Meets with clients at other agency clinic locations as necessary. * Follows standard operating procedures and AIDS Foundation of Chicago * Performs other duties as required or assigned * COVID-19 vaccination and negative TB results are conditions of employment. * Credentialing and Privileging required for this position. Implementation of services in the plan may take place individually or in groups. * Consistently share best practices across agency and networks, and standardized information and resources * Works collaboratively with CCHC leadership and strategy leads to support the enterprise in achieving strategic planning and quality strategy goals. * As a subject matter expert, the Director supports efforts in business development including the design and implementation of various BHI programs on behalf of CCHC * Supports implementation of behavioral health integration programs across CCHC. Successful implementation includes the proposal of standardized processes, quality and performance measurement. * Works by incorporating Population Health strategies, with clinical directors, providers, care and case management teams and community health workers in quality and best practice standards to ensure BH is integrated into care management and practice support functions. * In collaboration with CMO, develops training materials and implementation plans for working with populations around best practices in management of populations with BH and chronic medical needs, information around BH conditions, BH resources, etc. * Provides technical support in an effort to maximize collaboration with health systems, primary and specialty care practices, healthcare payers and other health, behavioral health and nonprofit sectors. * Aligns the BHI program goals with the priority goals of CCHC * Assists in planning and facilitating webinars and onsite trainings and attending external collaboration meetings as required. * Monitor and evaluate team schedules, and productivity templates * Maintain and submit weekly/monthly statistical reports related to program services performed (service referrals received, [internal and external] depression screens, domestic violence screens, individual counseling sessions conducted, etc.) * Document all client encounters and contacts made on behalf of clients utilizing prescribed forms; develop, complete and submit weekly progress reports. * Assist in an on-going appraisal of the program, which will result in continuous improvement in the quality of the program and in the development of new services. * Participate in multi-disciplinary staffing program and clinical meetings. * Perform other duties as assigned Employee Benefits offered to Fulltime Staff * Blue Cross Blue Shield Medical Insurance * Blue Cross Blue Shield Dental and Vision Insurance * Supplemental Benefits * Life Insurance (Provided by the company)
