POSITION SUMMARY: Under the direction and supervision of the Director, Business Planning and Regulatory Affairs, interpreter/translator provides interpreter and translation services for participants,
as requested or necessary.
DUTIES / RESPONSIBILITIES:
1. Provides interpretation services, including medical terminology
2. Regular document translation of non-legally binding documents, e.g., training, medical appointments, and participant communication
3. Works directly with team members at On Lok and participants
4. Coordinates scheduling at different centers where services are needed
5. Fulfills all administrative requirements, including record keeping, participating at meetings, satisfaction surveys, etc.

POSITION SUMMARY: Under supervision of Program Manager or designee, the Administrative Assistant II is responsible for achieving specified administrative objectives and performing a broad range of administrative and clerical duties.
DUTIES / RESPONSIBILITIES:
* Assists the Program Manager with administrative and clerical support.

PRINCIPLE RESPONSIBILITIES: The Medicare Field Sales Agent is responsible for compliantly achieving established monthly, quarterly, and annual Medicare Advantage (D-SNP) new enrollment and retention goals. Consistently enrolling self-generated, Partner generated, and company generated Medicaid conversion and marketing leads is essential to this role's success. The agent must work in collaboration with their team members and other Alameda Alliance for Health (AAH) department personnel to develop and execute an effective growth and retention strategy. A key part of that strategy will require them to work effectively and leverage relationships with providers, community organizations, and other influencers. Their engagement will take the form of participation in or sponsorship of local community events, individual provider co-marketing programs and targeted member outreach activities in conjunction with community partners.
Medicare Field Sales Agent will report to the Manager of Medicare Sales and Retention. The incumbent will be assigned specific responsibilities as determined by their manager based on opportunities identified related to product offerings, geographic or other market segmentation criteria.
The agent will be expected to generate new enrollments and achieve established retention targets through consistent and thoughtful member and prospect engagement activities. The agent must consistently complete proven "best practice" based activities to develop and manage their territories and/or assignments and provide regular status reporting at predetermined intervals via the sales departments established activity and performance tracking system. The agent will be expected to comply with all CMS, DMHC, and DHCS guidelines and quickly report any potential issues to their manager or Chief Compliance Officer.
Principle responsibilities include:
* Expected to achieve all established enrollment and retention objectives.

* Contacts: Manage the enrollment application process for all new members as established by Sales Leadership, CMS, and the Compliance Department. Work closely with Broker Agencies to recruit, train, and motivate them to enroll members, if applicable.
* Conflict resolution: When member or prospect issues arise, respectfully capture the available information and quickly relay it to appropriate AAH department and personnel.
* Member Communications: Provide feedback and collaborate with the appropriate department related to current materials or share insights or information on any changes that could improve the member's experience.

POSITION SUMMARY: Under the direction and supervision of the Lead Cook or Nutrition Services Manager, the Kitchen Helper coordinates meal services in the Day Health Center.
DUTIES / RESPONSIBILITIES:
Assists cooks to prepare center meals and other foods as necessary to complete meals. Portions individual items as necessary and readies any special requests prior to service. Serves food to participants in the dining room and dishes. Packages and ships meals and supplements to be delivered to participants at home.

POSITION SUMMARY:On Lok is seeking a dynamic Donor Relations Officer (DRO) to help drive philanthropic support in the south and east bay to advance its mission to empower seniors to age with dignity and independence in their communities. Reporting to the Chief Development Officer (CDO), the ideal candidate is a key frontline fundraiser based in the south or east bay and a relationship builder with a track record of donor engagement and revenue growth. POSITION SUMMARY: Under the direction and supervision of the Transportation Coordinator, the Driver will provide transportation services to all participants and make any deliveries and pickups as needed or assigned.
DUTIES / RESPONSIBILITIES:
1. Safe operation of vehicles (includes proper use of wheelchair tie down, use of lift, etc.) while transporting participants from their homes to the Day Health Centers and community appointments.
2. Ensure participant and vehicle safety (van maintenance, pre-trip inspections, use of seatbelts, body mechanics, transfer training, and wheelchair safety).
3. Provides timely and safe transport of participants, meals and supplies as scheduled or requested (between home and center, and community appointments)

POSITION SUMMARY: This position has a matrixed reporting structure to the Program/Center Manager and Center Support Supervisor, or may report to a Reception Supervisor if applicable. POSITION SUMMARY:
The Social Worker provides direct case management services to On Lok participants. The primary role of the MSW is to assist participants and their family/caregivers to cope with the social and emotional issues and practical arrangements related to maintaining their independence. With support from the Social Work Lead, the MSW serves as an integral member of the Interdisciplinary team (IDT) providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of participants and their families.
DUTIES / RESPONSIBILITIES:
Clinical Practices and Case Management
Utilize independent judgement and discretion to provide case management to a panel of participants.
Conduct initial and routine psychosocial assessments on new and existing participants in order to develop, implement and update individualized care plans and interventions.
Partner with caregivers and support system to develop care plan and deliver services.
Identify risks and other psychosocial problems.
Identify service needs of each participant; advocate, refer, coordinate and follow up in a timely fashion.
Provide individual counseling.
Facilitate family conferences.
Reach out to the larger community to meet participants' specific needs.
Assess needs and complete discharge planning when a participant dis-enrolls from On Lok.
Facilitate participant and/or caregiver groups and participate in special events such as weekend caregiver workshops.
Interdisciplinary Team Member Roles and Responsibilities
Act as the main liaison between participants, caregivers, the team, and the community.
Work with relevant departments such as Health Plan and other community agencies (e.g. RCFE, NF, DAAS)
Adhere to all ethical and legal standards of social work practices.
Comply with all On Lok policies & procedures including but not limited to PACE, safety, and confidentiality requirements.
Fulfill all documentation requirements in a timely and accurate manner.
Actively participate in team meetings, advocate for the needs of participants and their caregivers, compromise differences among team members, support and carry out team plans.

POSITION SUMMARY: Under the direction and supervision of the Program Manager, the Center Supervisor provides operational support to the PACE center by directing and supervising the administrative support staff and the center geriatric aides.

POSITION SUMMARY:
The Rehabilitation Aide provides functional maintenance therapy services to On Lok participants under the clinical supervision of Physical and Occupational Therapists. POSITION SUMMARY: Under the direction and supervision of the Supervisor/Manager, the Caregiver 2 provides personal care services to On Lok participants receiving home care and/or day center services. At On Lok, Caregivers provide the same services as a "Personal Care Attendant" position referred to in the PACE regulation (42 CFR Part 460).

POSITION SUMMARY:
The Rehabilitation Aide provides functional maintenance therapy services to On Lok participants under the clinical supervision of Physical and Occupational Therapists. This role may report directly to the Program Manager, Center Support Supervisor or a Clinical Specialist, depending on the Center configuration.

Maintain the tidiness and cleanliness of therapy equipment and devices according to agency cleaning protocols.

Demonstrate safe practices by utilizing proper body mechanics, adhering to universal precautions, and maintaining environmental safety.
