Job DescriptionWe are seeking a Part-Time/PRN Registered Nurse (RN) to provide high-quality, in-home care to patients across far South and Western suburbs of Chicago, specifically Aurora, Naperville, Plainfield, Bolingbrook, Minooka, Channahon, Joliet, and Lockport.

What You'll Do
Conduct in-homenursing assessments and ongoing patient evaluations
Perform initial start-of-care and re-certification visits as needed
Accurately complete OASIS documentation
Provide Skilled nursing care including wound care, IV line care, and maintenance
Educate patients and caregivers on disease processes, medications, and plans of care
Monitor patient progress and promptly report changes to physicians
Coordinate care with therapists, HHAs, and other disciplines
Supervise HHA field staff as appropriate
Ensure compliance with Medicare, agency policies, and clinical standards
Complete documentation and paperwork in a timely, efficient manner
