A Registered Nurse II (RN II) is a fully proficient RN who provides nursing care in a skilled and effective manner. + Observes signs and symptoms of illness, reactions to medications/treatments, general behavior, and/or general physical condition.
+ Demonstrates refined assessment skills, integrating appropriate data sources to assess the signs, symptoms, reactions, behaviors, or general appearance to determine normal versus abnormal characteristics.
+ Plans and implements individualized patient care based on observations. Implements appropriate reporting, referrals and care in accordance with standardized procedures. Initiates emergency procedures when indicated.
+ Provides care to special patient populations and patients with diverse cultural backgrounds. Identifies patient's readiness for learning and ability to follow directions/instructions and give consent.
+ Demonstrates knowledge and is able to identify and make special adjustments as required to the specific populations needs, including cultural, spiritual, age, psychosocial, communication, gender, sexual orientation, economic, education, family and condition needs.

Training & Advancement
Upon commissioning, Nurses who are new to the Navy are required to attend Officer Development School (ODS) in Newport, RI. ODS is a five-week program designed to introduce you to Navy culture. Nurses commissioned through a school ROTC program may not need to attend ODS.

Qualifications & Requirements
A degree from a four-year college or university is a minimum educational requirement to become a Commissioned Officer. You must also attend Officer Training. There may be exceptions to the degree requirements based on extensive service experience. Additional qualifications include:

Part-Time Opportunities
When serving part-time as a Navy Reserve Nurse, your duties are carried out during your scheduled drilling and training periods. During monthly drilling, Nurse Corps Officers in the Navy Reserve typically work at a location close to their homes.
For annual training, you may serve anywhere in the world, including locations in the U.S., at bases overseas, or in areas where humanitarian needs are great.

Job Overview
The LVN Supervisor is responsible for the day-to-day back office operations of the clinic by overseeing the employees in providing nursing care to the clinic patients, as well as implementing and assuring quality medical management and cost containment health services to patients. The Supervisor oversees planning, development, implementation, and evaluation/improvement of clinical care services/programs, which include patient experience, patient safety, HEDIS, and value-based care quality measures, financial goals, etc. The supervisor implements, maintains, and evaluates clinical and patient care practice standards. The Supervisor works with the Site Medical Director, Clinic Director, RN Manager and/or Regional Nurse Director and the Administrative leadership team to achieve clinic quality, patient satisfaction and financial goals and ensure all staff are up to date on clinic objectives using quality boards, presenting progress on clinic objectives at staff meetings, etc. Lastly, the Supervisor provides leadership to the nursing team and support as a mentor to staff.
Minimum Requirements
* Graduation from an accredited LVN school, coupled with a Current LVN license issued by the State of California required.
* A minimum of 3 years' experience providing patient care in an outpatient or inpatient setting.
* Current IV Certification preferred (will be required to complete within 6 months of employment).
* One year of supervisory experience preferred.
* Prior experience with an Electronic Health/Medical (EHR/EMR) system is required.
* A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Pay for this job starts at $77,126.00 annually

Job Purpose
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team providing exceptional care for elderly participants in the Program of All-Inclusive Care for the Elderly (PACE). The LVN will work as part of a multidisciplinary team to ensure high-quality healthcare services for older adults, promoting their health, independence, and well-being.
Duties and Responsibilities
Collaborate with physicians, Registered Nurses (RNs), social workers, and other healthcare professionals to develop and implement individual care plans for participants.
Assist with personal care tasks as needed, such as bathing, dressing, and mobility assistance.
Document all nursing activities and care provided in the electronic health record (EHR) system.
Support care coordination to ensure participants' needs are met in both the home and community settings
Participate in regular team meetings and case conferences to ensure comprehensive care delivery.
Adhere to all regulatory requirements, policies, and procedures to ensure compliance with PACE standards.
Coordinates the medical care of participants in assigned program, clinic, or service.
Performs and documents developmentally appropriate physical assessments.
Evaluates participant data and recognizes normal and abnormal findings.
Uses critical thinking and problem solving skills to work with participant and family to ensure an appropriate plan of care.
Conduct home visit to evaluate participant's care assessments.
Participate in Interdisciplinary team meetings and inform the IDT team for any changes in condition of the participants.
Evaluates and documents participant/family responses to interventions and treatment protocols or guidelines.
Coordinate home care for participants and manage home care vendor.
Coordinate necessary Dural Medical Equipment (DME) for participants and manage DME vendor.
Develop and implement policy and procedures for home care services.
Response to any concerns or feedback from participants and family members.
Serve as a liaison between CSC and Home Care Vendor to coordinate all cares and changes for participants.
Other duties as assigned.
Qualifications
Education:
Current and valid LVN license in the state of California
Experience in geriatric care, home health, or long-term care preferred.
Knowledge of chronic conditions, medication management, and aging-related health issues.
BLS certification (Basic Life Support) required.
Skills and Knowledge:
Knowledge of PACE program preferred.
Ability to work without close supervision or professional guidance and to exercise independent judgment.
Knowledge of physical, mental and social needs of frail older adults.
Able to manage changing priorities per participant needs.
Demonstrates necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile.
Ability to communicate effectively with elderly individuals and their families, showing empathy and understanding.
Excellent organizational skills, attention to detail, and ability to work independently as well as part of a team.
Fluent in Cantonese or Mandarin required
