At Hi-Desert Medical Center (HDMC) we've been caring for our community for over 40 years. Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests
The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms:
Subacute care: This is the highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care.
Skilled nursing care: We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures.
Long-Term Care: We provide long-term care to residents who require assistance with personal and medical needs.
Position Summary
This role provides direct clinical patient care.
The role will assume responsibility for assessing, planning, implementing direct clinical care to assigned patients on a per shift basis, and unit level.
The role is responsible for supervision of staff to which appropriate care is delegated.
The role is accountable to support CNO to ensure high quality, safe and appropriate nursing care, competency of clinical staff, and appropriate resource management related to patient care.
Shift: Rotate. Every other weekend required.
Hours: 7:00pm to 7:30am and 7:00am to 7:30pm
Education:
Required: Graduate of an accredited School of Nursing
Preferred: Academic degree in nursing (Bachelors or Masters degree)
Certifications
Required: Must be currently licensed, certified or registered to practice profession as required by law, regulation in state of practice or policy. CPR

Primary Duties
Conducts complete assessment for each patient following established guidelines/protocols to ensure and provide comprehensive patient care. Demonstrates ability to assess mothers and infants.
Develops plan of care, based upon assessment, or the individualized bio-psychosocial needs of assigned patients, incorporating the medical treatment plan and the nursing diagnosis.
Implements the plan of care in accordance with Hi-Desert Medical Center's standards of care, policies and procedures and the Nurse Practice Act.
Evaluates the effectiveness of nursing and medical interventions and demonstrates ability to change intervention strategies when expected outcomes are not met.
Documents care in a concise, accurate, legible, and timely manner, which reflects the extent of care given and responses to therapies.
Demonstrates effective interpersonal skills. Communicates appropriate information in an accurate and timely manner.
Utilizes a broad knowledge base to examine patient's needs and anticipate potential patient care problems.
Demonstrates appropriate priority setting and decision-making. Familiar with and able to implement all department specific protocols, procedures, and standards of care as detailed in Department Manual.
Assess patient learning needs and incorporates educational goals into the plan of care.
Able to use all department equipment correctly and safely. Maintains a clean and safe environment at all times to facilitate efficient patient care.
Assumes responsibility for own professional practice. Pursues growth opportunities. Participates in nursing seminars, committees, in-services, and department meetings.
Demonstrates ability to positively influence others' behavior by serving as a role model and continually striving to upgrade standards.
Assumes charge nurse duties as assigned.

The Perinatal Nurse is a Registered Professional Nurse assigned to the Perinatal Services Unit, who is responsible for the direct delivery of patient care using the nursing process of assessment, planning, implementation, and evaluation, to a specific group of ante partum, laboring and delivering, recovering, and post partum mothers, as well as their newborns. The Perinatal Nurse coordinates care in collaboration with the family, significant others, physicians, and other health care providers. The Perinatal Nurse is trained and able to initiate, modify, or stop treatment when appropriate and according to established protocols, even when a physician is not present. About the Job
Perform physical examination and establish medical history to determine donor suitability
Build rapport with donors to ensure overall customer satisfaction
Ensure donor and staff confidentiality
Responsible for donor awareness to potential hazards
Provide donor education regarding general health and provide counseling regarding unacceptable test results
Evaluate & manage donor injuries and adverse events
Perform evaluations of any history of illness or medications to ensure continued donor suitability
Assist in employee training

Job Requirements
Educated and currently certified/licensed in the state of employment and according to state requirements as a Registered Nurse.

Pay scale of $39.00-42.00 per hour, depending on training, education and experience. This position is eligible to participate in up to 6% of the company bonus pool. The Transitional Care nurse (TCN) will ensure the coordination and continuity of healthcare as patients transition between different locations or levels of care. The TCN will assist with complex patients, focus on high hospital utilizers, chronically ill and educate patients and family caregivers to determine the root cause to poor health outcomes that may lead to readmission to the hospital. The TCN will work with the Manager of Case Management, Care Managers and hospitalists to determine a safe discharge plan and ensure patient's continuity of care upon discharge.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Responsible for post discharge contact on a case-by-case concern with Medicare, other payers, and other identified complex patients, observation and inpatient, to determine that a safe discharge plan has been fully implemented.
Works with the Manager of Post-Acute Services Care Networks, Care Management Manager, Social Work and other members of the CM team to identify patients requiring follow up assessments and identification of criteria for patients requiring follow up phone calls or other contact.
Assess identified cases and utilize the information and available benefits/resources to assist the patient, family/caregiver and provider.
Document all activities specific to interventions for patients contacted in the electronic medical record.
Coordinates services in an effort to provide integrated health services for each patient and provide benefit and health information to each patient so they are able to make informed health decisions.
Assesses patient needs and make referral to any needed follow up services such as but not limited to MLKCH PDC, community resources, primary care provider, disease management, or other services.
Promote the mission and core values of MLKCH.
Document each call with all pertinent information along with interventions performed in a call log.
Acts as a consultant for the Transitional Care Navigator Lead or other members of the CM team.
Participates in performance improvement projects, initiatives and performs data collection for measurement of projects as assigned.
Works closely with Post-Acute Services to ensure that patients in settings other than home are appropriately placed post recuperative.
Has authority to add or extend services as needed to prevent readmission to MLKCH.
May participate in interdisciplinary discharge planning teams to ensure smooth transition from inpatient to outpatient services.
Meets regularly with CM Leadership & Post-Acute Network Manager to identify opportunities or trends to improve patient care.
Acts as CM with any recently acquired insurance; communicates and educates patient and family as needed in obtaining assistance from PCP or PDC.
Must utilize Translator assistance devices as needed to improve communications with patients and families.
Work with management to identify and vet potential post-acute service providers, as well as complete site visits to skilled nursing facilities with management to ensure MLK funded patients are receiving all necessary post-acute services.

POSITION REQUIREMENTS
A. Education
BSN required.
CCM or ACM certification within 2 years of hire.
B. Qualifications/Experience

WORK ENVIRONMENT
Nurse Corps Officers may serve at any one of more than 250 Navy and medical facilities around the globe, from Hawaii to Japan, Germany to Guam and Washington, D.C., to Washington state. As a Navy Nurse, you could work at one of the highly acclaimed National Naval Medical Centers in Bethesda, MD, Portsmouth, VA or San Diego, CA. Or you could provide medical support aboard one of two dedicated hospital shipsthe USNS Comfort and USNS Mercy. Additional opportunities are available on surface ships, with aircraft squadrons or even with the Fleet Marine Force.
QUALIFICATIONS AND REQUIREMENTS
To become a Commissioned Officer in the Nurse Corps, qualifications include:
U.S. Citizen between the ages of 18 and 41
Currently licensed and practicing nursing in the U.S. (new graduates must obtain a license within one year of beginning Active Duty service)
In good standing (as a student or graduate) with a CCNE-accredited U.S. education program granting a
Bachelor of Science degree

Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests
The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms:
Subacute care: This is the highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care.
Skilled nursing care: We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures.
Long-Term Care: We provide long-term care to residents who require assistance with personal and medical needs.
Position Summary
This role provides direct clinical patient care.
The role will assume responsibility for assessing, planning, implementing direct clinical care to assigned patients on a per shift basis, and unit level.
The role is responsible for supervision of staff to which appropriate care is delegated.
The role is accountable to support CNO to ensure high quality, safe and appropriate nursing care, competency of clinical staff, and appropriate resource management related to patient care.
Shift: Rotate. Every other weekend required.
Hours: 7:00pm to 7:30am and 7:00am to 7:30pm
Education:
Required: Graduate of an accredited School of Nursing

Primary Duties
Conducts complete assessment for each patient following established guidelines/protocols to ensure and provide comprehensive patient care. Demonstrates ability to assess mothers and infants.
Develops plan of care, based upon assessment, or the individualized bio-psychosocial needs of assigned patients, incorporating the medical treatment plan and the nursing diagnosis.

Able to use all department equipment correctly and safely. Maintains a clean and safe environment at all times to facilitate efficient patient care.

The Perinatal Nurse is a Registered Professional Nurse assigned to the Perinatal Services Unit, who is responsible for the direct delivery of patient care using the nursing process of assessment, planning, implementation, and evaluation, to a specific group of ante partum, laboring and delivering, recovering, and post partum mothers, as well as their newborns. The Perinatal Nurse coordinates care in collaboration with the family, significant others, physicians, and other health care providers. The Perinatal Nurse is trained and able to initiate, modify, or stop treatment when appropriate and according to established protocols, even when a physician is not present.
