The Wellness Director plays an integral role in upholding American House's mission to enrich the lives of
those we serve. Our Wellness Directors supervise, educate, direct and support all Wellness team
within the community and serves as manager on duty in the
absence of the Executive Director.
Duties/Responsibilities:
• Overall implementation, delivery, and coordination of resident services at the community.
• Carry out supervisory responsibilities of all wellness team members in accordance with
American House's policies and procedures. These responsibilities include interviewing, hiring,
and training employees; planning, assigning, and directing work; appraising performance;
rewarding and disciplining employees; addressing complaints and resolving problems.
• Coordinate, along with the Executive Director, family/resident care conferences and serve as a
direct liaison between residents, families, and staff.
• Assess/evaluate potential residents for Assisted Living/Memory Care level of care utilizing
assessment tool. This may involve assessing/evaluating potential residents in their home or
another healthcare setting.
• Ensure compliance with all state regulatory requirements including mandatory reporting to
State Regulatory Body.
• Ensure that all incident reports are completed, filed, and that follow-up is conducted
appropriately. Completing investigations as needed for all complaints, grievances, and
incidents, and working with the Executive Director concerning appropriate responses and
corrective actions.
• Ensure that all staff follows established safety regulations, including fire protection, infection
control, blood borne pathogens, hazardous waste, and quality assurance programs.
• Maintain compliance with state regulations and policies for medication administration including
oversite of medication management, validation of timeliness and completeness of medication
records, completing required responsibilities for Medication Aide programs via supervision,
delegation, training, and ongoing competency checks, where applicable.
• Provide marketing support within the community by participating in tours and discussions with
potential residents and their families about wellness offerings and clinical needs of the prospect.
• Maintain high level confidentiality of all information related to the role
• Create positive employee relations through staff appreciation programs, recognition of excellent
performance, and retention through regular feedback systems and promotional incentive
programs.
• Monitor licensure and certification compliance for all Wellness staff.
• Provide written and oral reports to the Executive Director and Regional Wellness Director
concerning the operation of the Wellness Department.
• Lead coordination of health care services with other third-party providers to ensure continuity
of care.
• Collaborate with the Executive Director for budget, equipment, and supplies needs.
Required Skills/Abilities:
• 3 years of management experience in healthcare or service-related setting
• Physiological and psychological knowledge related to gerontology
• Ability to have reliable transportation to perform offsite assessments and evaluations for
prospective residents
Preferred Skill/Abilities:
• Experience working with older adults and individual with memory loss
• Bachelor of Science in Nursing (BSN) Degree preferred
Education and Requirements:
• Licensed Practical Nurse or Registered Nurse
• Must pass a pre-employment background check, drug screen, TB test and physical

* Assess individuals upon admission and per state requirements thereafter meeting the needs of the individuals served.
* Under the direction of the DON, identifies, coordinates and monitors the health care of the individuals served, especially those with complex medical needs at assigned sites.
* Administration of first aid as needed.
* Diabetic foot assessments.
* Assists with coordination of care and treatment for the individual served to ensure psychiatric, medical, dental, and optical appointments are scheduled as needed.
* Communicates and coordinates and physical health concerns and follow up treatment with physicians and psychiatrists.
* Performs medication administration and initial medical classroom training and ensure state requirements are being met.
* Educate Direct Support Professionals (DSP) on the use of scales, blood pressure, oximeter, thermometer, stethoscope, glucometer, wheelchair, and any other durable medical equipment or any other need that is new to the home that is ordered by a physician.
* Participates in medical on call rotation if required.
* Assists with development and revisions of medical policies and procedures.
* Audit the medication room, EMAR and EHR programs following state requirements.
* Educates staff regarding infection prevention and monitoring and overall physical health of individuals served.
* When needed, provide input/data/summaries as part of the Infection Control Plan.
Education and Qualifications:
* Valid RN license with a minimum of two (2) years previous experience preferred.
* Must pass criminal background check
* Must possess a valid Driver's License.
* Proficient in speaking, reading and writing the English language.
* Approved by state, federal and government entities to work within BSLS programs.
* Ability to use office equipment, including BSLS information technology systems.

Under the supervision of the RN, provides prescribed medical treatment and personal care services which are within the scope of practice of practical nursing to ill, injured, convalescent, and disabled persons as delegated, appropriate to the patient's level of acuity and in accordance with established policies and procedures of the department and organization. The LPN will assist the Registered Nurse in collecting data, accompanying patients and responding to patient/family requests for services. The LPN may pass medications and administer IV therapy. Employees providing direct patient care must demonstrate competencies specific to the population served.
High school diploma or general education degree (GED) and successful completion of an accredited program for Licensed Practical Nursing required.
Experience in a health care delivery setting preferred.
Licensed Practical Nurse in good standing with the State of Michigan
BLS certification by the end of the core orientation period
* Ability to demonstrate specific clinical competencies required to perform the functions of the LPN within a general
orientation period of approximately 3 months

Must be able to effectively communicate with patients, visitors, and all members of the care delivery team; speech is clear, easily understood and grammatically correct; writing is clear and legible, demeanor is comforting and positive.
Work which produces very high levels of mental/visual fatigue, e.g. CRT work between 70 and 90 percent of the time, and work involving extremely close tolerances and considerable hand/eye coordination for sustained periods of time.
Involves considerable standing or walking, regular lifting of light-weight objects (i.e., 20 pounds or less) and assisting with heavier tasks such as assisting in lifting and moving patients. Minor straining or fatiguing positions must be assumed, and some fatiguing physical motions are required.
Admission and Preparation
Admits patients to the emergency department and completes preparation tasks
Coordinates with charge nurse when patient needs exceed scope of LPN practice, when census and acuity are high, and as needed.
Documents all data gathered and care given. Follow additional ProHealth guidelines when appropriate. Communicates status of patient to the physician.

Prepares or examines food trays for prescribed diet and feeds patients.
Washes and dresses bodies of deceased persons.
Assists in delivery, care, and feeding of infants.
Assembles and uses such equipment as catheters, tracheotomy tubes, and oxygen suppliers.
Sterilizes equipment and supplies.
Inventories and requisitions supplies.
Utilizes computerized systems to acquire needed supplies and to enter patient data as defined by role.
Maintains a clean, orderly environment
Patient Education and Discharge
* Provides teaching materials to patients based on identified patient needs, guidelines for care, and in coordination with physicians and RNs; documents completely
* Follows through on discharge process, assists patient as needed with appointments for further definitive care, rechecks, and others; uses ProHealth guidelines and restrictions when appropriate. Completes entries in ExpressCare log for each patient.
