GENERAL DESCRIPTION: Performs substance abuse and mental health counseling work on an individual patient basis and in a group setting. Duties primarily consist of conducting and documenting services such as counseling, psycho-educational groups, psychosocial assessment, case management, and treatment planning. Counseling includes discussion of issues associated with patient's substance use and other co-occurring conditions to work toward a constructive resolution of those problems and ultimately toward recovery. May be required to use telehealth services to provide clinical screenings, assessments, and counseling.
SCOPE OF PRACTICE: Clinical services are limited to the following:

REQUIRED QUALIFICATIONS: Must meet at least one of the following criteria:

Certified master's degree level addiction professional (MCAP) certified by the Florida Certification Board
Certified addictions professional (CAP) certified by the Florida Certification Board

Valid driver's license

Become the Case Manager you always wanted to be
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans
Participate in planning for and the execution of patient discharge experience.
Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.

Qualifications
- License or Certification:

If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position.

- Minimum Qualifications:

How You Will Make a Difference
Therapists are responsible for providing individual and group therapy sessions for clients in a specialized intensive outpatient treatment setting. Position is responsible for assessing the therapeutic needs of the individuals and families, and for developing appropriate treatment plans. Program Therapists act as the primary contact for clients and families throughout the treatment process and serve as a client advocate with the treatment team. Position acts as a liaison with referral sources, clinical partners, and internal resources to coordinate care throughout the treatment process.
Position Responsibilities
Assists in identification of treatment/contract goals, completing the initial psychosocial assessment.
Responsible for maintaining contact with the assigned client's referring/outpatient therapist during the course of the client's stay and the family when/if appropriate.
Completes and updates treatment plan for assigned clients.
Assist in providing clinical updates to the insurance company's Case Manager.
Meets with clients two to three times weekly, to help explore identified issues and behaviors in relationship to current symptoms.
Leads and participates in group meetings, meals and other activities in the milieu.
Provides comprehensive understanding of current behaviors and relational dynamics while using a variety of therapeutic modalities.
Attends the weekly team meetings and share observations, comments and important information about clients with the treatment team and provides input into the development of the weekly schedules for all of his/her assigned clients.
Responsible for proper charting and ensuring that all documentation is completed in an accurate and timely manner.
Assists in meal/snack supervision as assigned and attends special occasion activities for the clients.
Assists with discharge planning, including a written discharge summary and obtaining referral sources and necessary support systems once the client is discharged.

What We're Seeking
Position requires a master's degree in social work, marriage and family therapy or psychology. Position requires a minimum of one (2) years' experience in a behavioral healthcare setting, preferably with clients with mental health and substance use disorders, strongly preferred.
Doctoral level preferred (License not required)

The Counselor I/II works as a member of the treatment team, coordinating program services, and maintaining the well-being of clients and the integrity of the program.
QUALIFICATIONS: Following are the qualifications required to perform the essential functions of this position. Qualifications may be subject to modification based in the Americans with Disabilities Act. Bilingual abilities may be required within certain BHS programs.
Counselor I - High school diploma or equivalency. Registered and enrolled in a state-recognized chemical dependency counselor certification program or progress towards AOD, with six months substance abuse experience or Bachelor's and registered with state or nationwide approved credentialing body (i.e. CCAPP, CADTP).
Counselor II - High school diploma or equivalency. Required current certification number issued from a statewide or nationwide credentialing body (i.e. CCAPP, CADTP) received upon completion of passing the California Certification exam. GENERAL DESCRIPTION: Delivery of case management services to an adult population related to substance use and mental health.
