Competitive compensation and up to $25k sign on/retention bonus depending on experience. The Immediate Care Clinician will provide primary care to patients following accepted standards and protocols and will function as part of a team, seeing patients on a scheduled and walk-in basis and direct their clinical care together with other members of the care team. Provide direct patient care including pre-operative care, post-hospital follow-ups, physical exams (up to four physical exams per day, 30 minutes per exam) and urgent care, as well as telehealth care as needed. The Immediate Care clinicians will participate in regularly scheduled Immediate Care team meetings with the goal of maximizing the efficiency of the care process and designing and implementing standing orders to guide patient care and decrease clinical and administrative workload for Optimus' primary care clinicians. Job Qualifications/Requirements Strong interpersonal communication skills and the ability to work effectively with a wide range of patients in a diverse community. Ability to work independently, and to interact with colleagues and other staff in a team environment. Knowledge of clinical operations and procedures. Comprehensive knowledge of modern principles and techniques of general medicine and public health, as traditionally practiced. Ability to work well under pressure analyzes and evaluates individual member's needs, reach sound conclusions, and make appropriate recommendations. Must hold MD, DO, from an accredited medical school; BA/BS, MS preferred. Graduate of AMA accredited Physician Assistant or NP program. EXPERIENCE: 2 years of experience preferred in providing patient care as MD, DO PA, NP or Preference for board certification in internal medicine, family practice, or emergency medicine. LANGUAGE SKILLS: Bi-lingual Spanish preferred Licensure / Certification Current unrestricted Connecticut License to practice medicine as and MD, DO, PA, NP Active Federal DEA and Controlled Substance Registration For physicians -board certified in Specified Area of Medical Specialty.Certified PA or APRN Current CPR certification and ACLS certification required. Navigates clients through or around transportation, public benefits, childcare, and other barriers to successfully participate in MAT services. Assist in linkage to recovery support activities by removing personal and environmental obstacles. Refers individuals to appropriate medical, psychiatric, housing and/or behavioral health services utilizing designated collaborating organizations (DCOs) whenever possible to ensure rapid entrance into programs without delay. Works closely with colleagues and community-based providers to ensure clients and their families receive access to timely, high quality care and services, including follow up to ensure connection. Serves as a client advocate in obtaining necessary services. Documents all contacts; works collaboratively with the clinical team to assist in identifying and placing clients in the appropriate aftercare services that offers interventions tailored to their diagnoses. Participates in interdisciplinary continuous quality improvement activities on an ongoing basis; participates in weekly clinical meetings and individual supervision.
