Diagnose and manage ocular and visual conditions such as refractive errors, amblyopia, binocular vision disorders, glaucoma, diabetic eye disease, and anterior/posterior segment conditions within the optometric scope of practice. Prescribe and fit glasses, contact lenses, and other visual aids as appropriate. Provide patient education on eye health, disease prevention, and vision care. Manage ocular emergencies and refer patients to outside specialists when necessary. Collaborate with primary care providers and other HCD CHC staff to ensure coordinated, whole-person care. Maintain accurate, timely, and compliant medical records in accordance with HCD CHC standards. Participate in quality improvement, health education, and community outreach activities as assigned. Review incoming reports (e.g., x-ray, hospital and specialties reports, etc.) sign, date, and follow-up in a timely manner. Job Description The Optometrist will perform an in-person assessment with the Veterans to complete the required Disability Benefits Questionnaire (DBQ) for submission to VA for eligibility determination. The Optometrist will be responsible for providing complete high quality optometric/vision care services to patients and performing evaluation of the eyes and provide medical opinions. Duties include the following.
