Responsibilities
The Optometrist will provide general optometry services to the residents of nursing homes, assisted living facilities, and long term care facilities within an assigned territory.
Essential Functions:
Diagnose and treat refractive disorders.
Diagnose and treat ocular disorders caused by age-related illnesses.
Examines, diagnoses, treats, and manages diseases, injuries and disorders of the visual system, the eye, and associated structures.
Identifies related systemic conditions affecting the eye.
Enters patient information and completes required clinical documentation in our cutting-edge EMR system.
Mobile model treatment of patients requires daily travel to skilled nursing facilities.
Perform other duties as assigned.
Qualifications
Doctorate of Optometry (O.D.)
State professional license
Valid driver's license
TPA certified/Legend Drug Certificate
Ability to work independently on a daily basis.
Excellent written, verbal, interpersonal and organizational skills.

What You'll Do
a. Plans, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the patient's health. e. Works with CMEP leadership to develop and maintain the social service program, drafting supporting policies, procedures, and guidelines for the program. Develops and maintains a thorough knowledge of referral resources throughout the service area.
f. Maintains required documentation for all care management activities, and codes appropriately for services provided.
g. Maintains accurate and timely records including clinical and progress notes according to department policy and codes appropriately for services provided.
h. Develops working relationships with leaders in community resource groups to coordinate care across to the continuum, to offer patients options, tools and resources, and to represent CMU Health at meetings and on various committees that address the social needs of patients.
i. Supports CMEPs commitment to education by occasionally supervising social work students for internships or other educational opportunities.
j. Acts as resource person for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems.
k. Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.

POSITION SUMMARY:
Under the clinical direction of the Medical Director and administrative direction of the Chief of Health Services, provides outpatient psychiatric care to consumers and their families. ESSENTIAL DUTIES AND RESPONSIBLITIES:
Provides psychiatric oversight by reviewing and approving agency diagnostic evaluations, treatment plans and other assessments as required through funding sources and/or accreditation bodies in a timely fashion.
Complete psychiatric evaluations and medication reviews of assigned consumers in a timely fashion.
Prescribes medication where appropriate. May prescribe medication to a consumer while performing an on-call coverage for another Psychiatrist and/or Medical Director.
Orders and monitors laboratory and associated tests related to consumer's psychiatric care and needs.
Treats mental health issues involving the consumer and their families.
Participates in clinical program planning.
Maintain own professional development to ensure continuing improvement in providing quality care to their consumers and their families.
Interprets clinical findings, as needed, to staff, parents, teachers, family members, and any other consented individuals.
Utilizes a team approach to keep all members informed, is respectful of the time for all participants in completing services and documentation.
Consults with other professional staff and programs in the treatment of consumers and their family.
Prepares diagnostic reports in a timely fashion. Completes documentation in SCCMHA electronic medical record and required paper records as well as those required by the host agency record keeping systems.
Documents time in the SCCMHA electronic medical record, accurately reflecting services and amount of time spent. Records all communications and consultations.
Attends required in-services and provides in-services to staff as needed and requested. May participate in academic training of college students from any University affiliated with the agency according to policy.

Participates in peer chart reviews as assigned.
Serves as SCCMHA subject matter expert in the psychiatric care of consumers and families.
