JOB TITLE: Managing Optometrist Provide stellar patient care through proper diagnosis and treatment of conditions and diseases of the human eye and visual system. DUTIES AND RESPONSIBILITIES: 1. Meet with clients and /or family members and other providers to complete a psychiatric/ mental health assessment. Address psychiatric/ mental health needs identified in assessment. Diagnose clients and treat those who have a mental illness. Prescribe medication as necessary with consideration of allergies, substance use, current medication, medical history and physical status. Educate individuals and family members regarding medication and its actions, effects, and side effects so that they can effectively participate in decisions concerning medication that is administered/dispensed to them. Evaluate the effectiveness of the medication and of a wide range of therapeutic measures. Assist individuals in obtaining prescribed psychotropic medication, when needed, and provide follow-up as needed. Notify and assess clients for possible side effects of medication. Consult with clinical staff at Phoenix Rising regarding care of clients in a multidisciplinary approach. Write reports for insurance carriers as well as Social Security Administration and/or other forms as needed by the clients in a timely manner Document and record information per policy and procedures of Phoenix Rising.
