Essential Functions
Receives and screens all home health orders based on Medicare guidelines for home health eligibility. Coordinates with collaborating physicians on all orders and ensures that the physician's signature is documented on all orders. Reviews and holds home health plan of cares and certifications for billing purposes. Processes and links orders into the patient's medical record. Ensures accurate and timely delivery of orders to proper agencies. Communicates by phone and email with home health agencies about pending orders, missing orders, and other issues that may arise. Encourages agencies to participate in exchanging orders electronically. Coordinates patient care as ordered/instructed by the provider, including but not limited to: placing lab and radiology orders, arranging referrals to specialists and home health agencies, arranging outpatient testing, request medical records, and communicating with existing specialists. This also includes ensuring that appropriate and accurate medical information is provided to healthcare providers. Places urgent orders when requested by the provider. Tracks patient orders sent to agencies to ensure orders are processed as ordered and results are received timely. Links patient documents to the appropriate patient charts in the EMR that include but are not limited to test results, DME orders, and home health orders. Sends documents to Providers in the EMR for review, as needed. Returns completed orders to agencies by fax, email, or other electronic method. Cross-train as back up for other positions as assigned. Organizes, attends, and participates in meetings with home health representatives to review processes and ensure timely return of orders. Utilizes critical thinking skills with any issues/concerns that may arise and determines solutions to the problems. Completes special projects as assigned. Attend Company and industry-related meetings, as required. Establishes individual goals which are aligned with the Company's business strategies and objectives. Functions as a contributing member of the department's team and other teams, as assigned. Performs other duties as assigned by executive leadership.
