Provides spiritual and emotional care for patients, their loved ones and team members on assigned unit(s).
Hours: Monday - Friday 12:30-9pm, with two Sundays per month.
Responsibilities
• Requires direct access to Protected Health Information (PHI). The PHI accessed is used for patient treatment, and healthcare
operations purposes through direct contact with patients, family members, patients' medical records/charts, computer applications,
and verbal reports from healthcare providers, and interaction with various hospital interdisciplinary team members.
• Documents interventions on the patient chart, including, but not limited to such information as referral source, spiritual assessments,
interventions, and follow-up/referral plan.
• Serves as a member of the interdisciplinary clinical team, participating in patient rounds, case reviews, ethics consultations, and
advanced directive consults.
• Provides crisis intervention and bereavement support for persons experiencing suffering, tragedy, or loss.
• Performs requested sacramental services, applicable chapel services, and/or arranges for the provision of these services by
community clergy.
• Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state
and local standards.
• Maintains compliance with all Orlando Health policies and procedures.
Qualifications
Education/Training
• Master level theological degree is required, with a Master of Divinity being the preferred degree. Master level equivalent
theological degrees may be considered.
• Accreditation by the Association of Theological Schools preferred or a listing in The Database of Postsecondary Institutions and
Programs (ope.ed.gov/dapip/)
• Four (4) units of Clinical Pastoral Education for full time, two (2) units of Clinical Pastoral Education for part-time, one (1) unit
of Clinical Pastoral Education for pool. All units must be recognized by ACPE, APC, NACC, NAJC, CPSP, CPE-I, or ICPT.
Licensure/Certification
Ordination, licensure, or commissioning and endorsement as a chaplain by a religious body listed in the US Defense Department list
of Recognized Religions.

Masters Theological degree (e.g. Master of Divinity (MDiv/equivalency as established by the Association of Professional Chaplains (APC) from an accredited seminary of the Council for Higher Education Accreditation (CHEA) Minimum of four units Clinical Pastoral Education from an Association of Clinical Pastoral Education (ACPE) approved center with the exception of faith group specific applicants for whom CPE is not required. In these cases, the applicant must complete four ACPE units within two years of hire and become Board Certified within four years of hire. An applicant of a current ACPE Resident with three completed units of Clinical Pastoral Education must complete the fourth CPE unit within 1 year of hire. Strong verbal and written communication skills required, including computer skills. Electronic Medical Record (EMR) experience preferred. Two years working experience in a healthcare setting preferred. Preferred education: Master's/Advanced Degree in related fields (e.g. Master's in Public Health (MPH; advanced research degree and Masters in Philosophy/Ethics which is common in seminary/religious/ethical education training). Endorsement for service as a Chaplain by appropriate faith community. Board Certification or certification-eligible (BCC) within one or more of the following: Association of Professional Chaplains (APC), National Association of Catholic Chaplains/United States Catholic Conference of Bishops (NACC/USCCB), the Association of Clinical Pastoral Education (ACPE), or the Association of Jewish Chaplains (AJC). Responsibilities
Enhabit Home Health & Hospice is searching for a Chaplain with hospice experience to join our team. The Chaplain is a member of the interdisciplinary team and reports to the Hospice Branch Director.
Providing direct spiritual support and/or counsel to patients/families in keeping with patients/families' beliefs.
Working with staff, clergy and community groups to enhance their sensitivity to the spiritual needs of patients/families and reporting on services as indicated.
Providing consultation and education for patients/families and interdisciplinary team members.
Providing bereavement follow-up services as defined by Hospice.
Maintaining proper records of visits to patients/families.
Performing occasional liturgical assignments such as the annual Service of Remembrance and monthly memorial service when indicated.
Qualifications
Education and Experience (ESSENTIAL):
Must be an individual who, by ordination or by ecclesiastical endorsement from the individual's denomination, has been approved to function in a pastoral capacity.
Must have demonstrated experience in working with patients and families dealing with life-threatening illness and death.
Education and Experience (DESIRED):
Training in clinical pastoral education, meeting the requirements for the college of chaplains, may be considered in lieu of ordination or endorsement.
Qualifications:
Must be able to effectively communicate, both orally and in writing.
Must have exceptional customer service skills and be able to communicate well with a diverse group of stakeholders.
Must be organized and able to perform multiple, critical tasks simultaneously and frequently.
Must have an understanding of issues related to delivery of home care services.
Must have demonstrated ability to be open, sensitive, flexible, and ecumenical.
Must be able to function efficiently and in a positive manner within a high stress environment.

Role:
The Chaplain provides a wide range of spiritual services to patients and families enrolled in any of the Company programs. The Chaplain also provides spiritual support to the staff and volunteers of the Company.
Qualifications:
If hired after 5/26/2024, Master's degree required.
If hired prior to 5/26/2024, Bachelor's degree in Divinity or related Degree from an accredited institution required.
Ordination or commissioning/endorsement from an ordaining body recognized by the Endorser's Conference, Veterans Administration that demonstrates the candidate has standing with the organization; ordination must be obtained within 12 months of hire
Documentation that demonstrates the candidate is in good standing with faith group or professional affiliation group with no ethical charges current or pending
One unit of Clinical Pastoral Education (CPE) accredited by the Association for Clinical Pastoral Education, Inc.; or in process within six (6) months of employment/position
Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system preferred; appropriate computer skills required
Health care experience preferred
Mobile Driver - Valid driver's license and automobile insurance per Company policy
Ability to travel and have reliable transportation
Must be able to physically access (i.e., navigate stairs and narrow spaces, endure heat and lack of air conditioning) any home, hospital, nursing home and/or assisted living facility (ALF)
Must have the endurance to manage demands of a hospice care field work position (i.e., time, travel, home/facility arrangements, stress, etc.)
Must be able to communicate clearly in English

Job Responsibilities:
Develops a spiritual care plan in conjunction with the interdisciplinary group based upon patient/family spiritual traditions and spiritual assessment results.
Provides appropriate spiritual care and initiates interventions necessary for resolving care plan issues.
Implements the spiritual care plan through personal contact and/or enlistment of Spiritual Care Volunteers, local clergy or other counselors as may be appropriate in each circumstance.
Monitors implementation of the spiritual care plan and reassesses as needed.
Maintains documentation in manner reflecting timeliness, completeness, accuracy, legibility, clarity of goal, assessment and plan.
Provides appropriate spiritual care in accordance with Company philosophy and policies, procedures, and standards of practice.
Acts as liaison between the Company and local clergy and congregations in the interpretation of the Company mission and programs.
Attends and participates in interdisciplinary team meetings, assists in developing care plans, facilitates understanding of spiritual aspects of care and provides team with spiritual care objectives.
Performs other duties as assigned.
