Daily monitoring of progress towards discharge plans and/ or need to alter discharge plan due to change in patient condition / family needs with a priority placed on those patients at highest risk for complication/ admission/ readmission. Educates patients/ families with chronic illness about evidence-based standards of care to include self-management strategies. Identifies support needs for patients and their families, develops action plan(s), and provides creative guidance in initiating and overcoming any self-management strategies. Educates patients and families about the health care system and facilitates relationship building between the various settings. Ensures patients have access to prescriptions, durable medical equipment (DME), and other services as identified. Process paperwork and complete clinical documentation.

Follow federal regulations and administrative tasks.
Develop individualized master treatment plans.
Follow up on referrals from outpatient providers.
Minimum qualifications for the ideal future caregiver include:
Master's Degree in Social Work, Counseling or a related field
Licensed Independent SocialWorker (LISW), or Licensed Professional Clinical Counselor (LPCC), or Licensed Professional Counselor (LPC) or Licensed SocialWorker (LSW) with no record of substantiated ethics or patient abuse reports

JOB SUMMARY

Psychiatric Emergency Clinician 2 Job Responsibilities:
Provides psychiatric consults to patients who present to the Emergency department onsite at Fairfax ER, via telepsych at other Inova ERs or Urgent Care facilities, or at IPAC walk in clinic.
Maintains therapeutic milieu to provide a safe environment for crisis stabilization. Assesses needs and formulates collaborative treatment recommendations.
Provides psychiatric consults to assess and diagnose mental health and/or substance use disorders as well as providing risk assessments.
Integrate assessments using information to develop a recommendation for care
May provide a range of brief therapeutic interventions including, group programming, individual crisis therapy, family support sessions and behavioral interventions to assist the patient in achieving the goals and objectives for stabilization.
Provides care coordination to ensure the patient and family receive and are referred to appropriate resources to optimize treatment outcomes.
Minimum Qualifications:
Education: Master's Degree, Ph.D., PsyD, EdD in Mental Health, Social Work, Counseling, Substance Use Disorders or Behavioral Health related field.
Experience: Minimum of two years' experience working in the psychiatric or mental health field with the seriously mentally ill.
Certification: BLS from the American Heart Association Upon Start. Crisis Prevention Institute Within 6 Months CPI in-person training for non-violent crisis intervention, receive CPI blue card within 6 months of hire. Vistelar within 6 months, after 9/1/25 Vistelar replaces CPI for new and renewed de-escalation certification.

Provides patient and family education regarding lymphedema management, home programs, and prevention of complications.
Ensures timely, complete documentation and accurate billing in accordance with departmental and organizational standards.
Adheres to the State Practice Act and Standards of Practice for Occupational Therapy or Physical Therapy, as applicable.
Maintains compliance with all Orlando Health policies, procedures, and regulatory requirements.
Maintains a positive and encouraging treatment environment that supports patient engagement and recovery.
Participates in performance improvement initiatives, program development, student supervision, safety, and infection control.
Demonstrates professional conduct, effective communication, and a commitment to patient-centered care.
Practices efficient use of time, supplies, and equipment.
Attends staff meetings, in-services, and required interdisciplinary care conferences.
Instructs and supervises supportive personnel as appropriate.
Education/Training Bachelor's degree or higher from an accredited program in Occupational Therapy or Physical Therapy.
Formal training and certification in Lymphedema Therapy required (CLT or equivalent).
Licensure/Certification Maintains current Occupational Therapist or Physical Therapist license in the State of Florida (temporary permit acceptable).
Maintains current BLS/HealthCare Provider certification.
Experience Lymphedema therapy experience required.
