Job Summary:
LPN Supervisor, Ambulatory, has operational and clinical supervisory responsibility and is responsible for overseeing the daily operations of the clinical staff within the department.
Responsibilities
Essential Functions
The Supervisor focuses on daily operations such as personnel supervision, compliance with organizational policies and procedures, facility management, patient scheduling and business-related reporting and functions.
Participates in the hiring, supervision, work assignments, coaching plans, and disciplinary actions of assigned team members.
Possesses a clear understanding of patient flow and processes. Reviews staffing requirements on daily basis. Makes changesto clinic schedule enabling good patient care.
Monitors workflow for completion of duties and conducts staffing and operations reports to provide communication to all team members, managers, and providers.
Identifies training needs for team members and facilitates development opportunities. This includes the evaluation of training, implementation, orientation, and participation of education programs.
Coordinates the purchase of specialized equipment and supplies utilized by the clinic assigned.
Demonstrates knowledge of standards and recommended practices for the assigned clinic.
Communicates appropriate information regarding the patient condition to the physician, ARNP, and/or PA, and other members of the healthcare team.
Able to collaborate effectively with physicians and administrative staff.
Assists physician(s) with creation of strategies for efficiency and growth of the practice.
Provides management reports to physicians and administration for review, practice planning, practice marketing, and completes practice development.
Presents a positive representation of corporate and departmental services.
Contributes to departmental effectiveness by reporting to duty on time, adhering to policy concerning breaks and mealtimes and completing duties in an efficient, effective, and timely manner.
Serves as a preceptor or mentor, to other LPN's and/ or Medical Assistants in the outpatient clinic. Mentors, coaches, and advises staff to maximize performance, comfortable seeking multidisciplinary consultation.
Provides department level orientation to new team members to include system software, telephone processes and protocols, office equipment usage, departmental policies and procedures, customer service expectations, job competencies, and coaching plans.
Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA, and other federal, state and local standards.
Ensures clinic complies with all safety & regulatory guidelines and Orlando Health policies and procedures.
Educates and supervises clinical and non-clinical staff.
Participates in nursing PI activities as assigned, under the guidance of an advance practice provider or RN.
Practices efficient use of supplies.
Maintains a clean, safe, and organized unit.
Knowledge of medical practice protocols and standards.
Skilled in documentation of patient care including reviewing for accuracy and completeness.
Support service line leadership with ensuring all clinic staff are compliant with safety and or regulatory guidelines
Qualifications
Education/Training
Graduate of an approved school of practical nursing.
Successful completion of a 30 hours approved IV therapy course may be required for specific clinics.
Licensure/Certification
Current LPN license in the State of Florida renewed every two years.
Maintains current BLS/Healthcare Provider certification
Experience
Three (3) years of directly related experience required.

RegisteredNurse (RN) Case Manager 1 Job Responsibilities:
Collects delay and other data for specific performance and/or outcome indicators. Assists in the collection and reporting of resource and financial indicators including acute and post-acute case mix, LOS, cost per case, excess days, resource utilization, readmission rates, denials and appeals. Collects, analyzes and addresses variances from plans of care and care paths with physicians and/or other members of the healthcare team. Uses concurrent variance data to drive practice changes and positively impact outcomes. Documents key clinical path variances and outcomes which relate to areas of direct responsibility (e.g. discharge planning, chronic disease planning).
Uses pathway data in collaboration with other disciplines to ensure effective patient management concurrently. Ensures safe care to patients by adhering to policies, procedures and standards within budgetary specifications including time management, supply management, productivity and accuracy of practice. Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency. Supports department based goals which contribute to the success of the organization.
Provides discharge planning and continuity of care for assigned patients in the acute and post-acute setting. Minimum Qualifications:
Education: BSN from an accredited school of nursing. If RN has an associate's degree (ADN); must complete BSN within 5 years of start date.
Experience: Requires a minimum of 1-year Case Management and/or Clinical Care experience.
Certification: Currently licensed as a RegisteredNurse in the State of Virginia or hold a privilege to practice in the State of Virginia under the Enhanced Nurse Licensure Compact (eNLC). Basic Life Support (BLS) for Healthcare Provider certification from the American Heart Association required upon start.
Preferred Qualifications:
One (1) year of previous inpatient case management experience is highly preferred.

Case Manager Certification (CCM) required within one year of eligibility
Six years of experience in Care Management, Utilization Review or Discharge Planning in a Healthcare setting
Demonstrated leadership skills
Strong analytical, data management and PC skills
