    Requirements Key Responsibilities: Monitor and advocate for the health and medical care of individuals served. Review, update, and maintain medical documentation and databases (including MyEvolv system). Conduct monthly nursing audits and report findings. Educate individuals served, families, and staff on health-related topics and policies. Attend and participate in residential staff meetings and administrative meetings. Visit assigned residential homes at least three times per week. Accompany individuals to complex or serious medical appointments as needed.
