Primary Purpose of Position: The Team-Based Care Registered Nurse (RN) is an integral member of the team-based care model, working in close collaboration with providers, medical assistants, behavioral health specialists, and other care team members to deliver high-quality, patient-centered care. The RN supports chronic and acute care management, patient education, preventive care, and population health initiatives in alignment with clinical and organizational priorities. Essential Functions: Direct Patient Care & Clinical Support Conduct nurse visits including blood pressure checks, wound care, injections, allergy shots, INR checks per standing orders, vaccine administration per standing orders or provider direction, and other visits per protocol. Perform triage using clinical protocols, assess patient needs, and coordinate same-day care when necessary. Exhibit high level of clinical acumen and decision making to assist in the overall care of patients' acute and chronic needs as part of a highly functioning clinical team. Provide medication education and reinforce treatment plans for chronic disease management (e.g., diabetes, hypertension, asthma). Perform medication refills per standing protocols. Assist with post-hospital discharge follow-up and care transitions. Conduct routine patient examinations and assessments according to scope of practice based on licensure and OCHS protocols. Provide the best patient experience possible; address patient's concern and questions, explain procedures, testing, and diagnosis thoroughly. Educate patient in the proper use of medical equipment, prescribed regimens and other nursing care regimens. Refer patient to outside sources, programs, or support groups, for additional education or referrals. Other related duties and projects, as required or assigned in order to ensure the needs of the Health Center and its patients are met. Other related duties and projects as assigned. Care Coordination & Panel Management Collaborate with providers and care team members to support panel management and ensure timely follow-up on labs, referrals, and preventive services. Participate in pre-visit planning with a focus on leveraging standing orders and written protocols to ensure patients are staying up to date on care gap and chronic disease management. Review dashboards and care gap reports to outreach to patients and schedule necessary visits. Participate in team-based huddles, case conferences, and interdisciplinary care planning. Team-Based Care & Quality Improvement Work collaboratively within the care team to ensure a seamless patient experience. Contribute to quality improvement projects and help implement population health strategies. Support coordination of specialty care, social services, and community-based resources. Documentation & Compliance Maintain patient records with accurate and timely documentation in the Electronic Medical Record (EMR) in accordance with OCHS policies. Ensure compliance with infection control standards, medication safety, and clinical protocols. Maintain patient privacy and confidentiality of information at all times in accordance with OCHS policies and HIPAA. Qualifications Qualifications: BLS/CPR certification required. Minimum of 1-2 years of clinical nursing experience preferred, ideally in a primary care or community health setting. Experience or desire to work in team-based care model. Bachelor of Science or Associates in Nursing from an accredited school of nursing by AACN. Current Registered Nurse (RN) license from the Massachusetts State Board of Registration. Maintain and enhance professional nursing skill and knowledge by attending continuing education programs. Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting. Excellent written and verbal communication skills - bi-lingual helpful. Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc. Excellent judgment and ability to solve problems in a timely manner. Ability to adhere to strict confidentiality standards. Demonstrated ability to work effectively both as a team member and independently. Demonstrated ability to work in a culturally diverse and inclusive environment. A day in the life of this role: The Registered Nurse at Outer Cape Health Service ("OCHS") is responsible for providing direct patient care based on the nursing process within assigned work periods. The incumbent functions as a member of a multi-disciplinary team to achieve patient outcomes consistent with a comprehensive plan of care. Summary of duties: * Patient Assessment and Triage * Conduct intake on walk-ins to identify need for urgent care * Assist in triaging patient telephone calls * Prioritize patient needs based on the severity of their condition. * Care Coordination: * Collaborate with other healthcare providers, including physicians, specialists, and allied health professionals, to develop and implement care plans. * Assist with the coordination of referrals and follow-up appointments with the assistance of the referral department. * Ensure smooth transitions of care between different settings, such as from hospital to home. * Patient Education and Advocacy * Educate patients and their families about health conditions, treatments, and preventive measures. * Provide the best patient experience possible; address patient's concern and questions, explain procedures, testing, and diagnosis thoroughly * Educate patients on the proper use of medical equipment, prescribed regimens and other nursing care regimens * Medication and Immunization Management * Educate patients on proper medication use and adherence. * Documentation and Record Keeping * Maintain patient records with accurate and timely documentation in the Electronic Medical Record (EMR) in accordance with OCHS policies * Document all care provided and any changes in patient condition. * Clinical Interventions and Procedures * Implement orders for EKG's, dressing changes, suture removal, ear lavages, splinting, injections, IV insertion, and IV infusions under the direction of a clinician. * Obtain required patient specimens and perform or request diagnostic laboratory tests * Promotion and Disease Prevention * Participate in population health initiatives and screenings. * Provide counseling on lifestyle changes to prevent illness and manage chronic conditions. * Quality Improvement and Safety * Participate in quality improvement projects aimed at enhancing patient outcomes and care processes. * Support and Supervision * Mentor and support other nursing staff and healthcare team members. * May be required to pick up Saturday shifts as needed to support clinic coverage * Crisis Management * Respond to medical emergencies and provide critical care support. * Participate in disaster preparedness and response activities.

The incumbent functions as a member of a multi-disciplinary team to achieve patient outcomes consistent with a comprehensive plan of care. Summary of duties: Patient Assessment and Triage Conduct intake on walk-ins to identify need for urgent care Assist in triaging patient telephone calls Prioritize patient needs based on the severity of their condition. Care Coordination: Collaborate with other healthcare providers, including physicians, specialists, and allied health professionals, to develop and implement care plans. Assist with the coordination of referrals and follow-up appointments with the assistance of the referral department. Ensure smooth transitions of care between different settings, such as from hospital to home. Patient Education and Advocacy Educate patients and their families about health conditions, treatments, and preventive measures. Provide the best patient experience possible; address patient's concern and questions, explain procedures, testing, and diagnosis thoroughly Educate patients on the proper use of medical equipment, prescribed regimens and other nursing care regimens Medication and Immunization Management Educate patients on proper medication use and adherence. Documentation and Record Keeping Maintain patient records with accurate and timely documentation in the Electronic Medical Record (EMR) in accordance with OCHS policies Document all care provided and any changes in patient condition. Clinical Interventions and Procedures Implement orders for EKG's, dressing changes, suture removal, ear lavages, splinting, injections, IV insertion, and IV infusions under the direction of a clinician. Obtain required patient specimens and perform or request diagnostic laboratory tests Promotion and Disease Prevention Participate in population health initiatives and screenings. Provide counseling on lifestyle changes to prevent illness and manage chronic conditions. Quality Improvement and Safety Participate in quality improvement projects aimed at enhancing patient outcomes and care processes. Support and Supervision Mentor and support other nursing staff and healthcare team members. May be required to pick up Saturday shifts as needed to support clinic coverage Crisis Management Respond to medical emergencies and provide critical care support. Participate in disaster preparedness and response activities. Qualifications What we need from you: Associate of Science in Nursing from an accredited school of nursing by AACN, BSN preferred Current Registered Nurse (RN) license from the Massachusetts State Board of Registration Basic Life Support (BLS) 1 year professional nursing experience preferred (ED, urgent care, or ambulatory/primary care setting) or comparable experience.
