Education & License Requirements: Current licensure in the State of Oregon as a Physical Therapist and a current CPR card. Minimum Qualifications & Experience: One year of inpatient or outpatient physical therapist experience is required. Must have current driver's license, insured vehicle for transportation, and a means of communication. Essential Job Functions: Evaluate patients, plan, and implement effective safety interventions based on physical assessment, clinical judgment, and psychological sensitivity. Understand the hospice philosophy and may provide care to dying patients and their caregivers. Utilize current standards of practice and follow departmental policies and procedures. Comply with applicable accreditation and regulatory requirements governing the practice of physical therapy. Maintain a high level of professional expertise and assume responsibility for professional growth and development. Collaborate with other home care services, including nursing, social work, and additional therapies, to provide patient care. Administer treatments and use therapeutic equipment; document treatment utilizing software system. Educate patients and their caregivers. Work independently and be adaptable to change. Demonstrate sound judgment and effective time management skills. Meet safety needs of patient. Communicate clearly, including verbal, written, telephonic, and electronic.
