Outpatient physical therapist job in Detroit, MI

Occupational Therapist Key Responsibilities:
Treatment Planning: Create and deliver treatment programs for students aged birth to 22 based on their IEPs or medical plans. Collaborate with the interdisciplinary team to set goals and accommodations. Oversee and support Occupational Therapy Assistants (COTAs) as needed.
Consultation & Instruction: Act as a district-wide resource by offering consultation, training, and support to educational staff and caregivers. Build and maintain positive relationships with students, parents, and staff, and coordinate with medical personnel to implement home programs.
Data Collection & Documentation: Conduct comprehensive evaluations of students' needs, perform initial and ongoing assessments, and complete annual and triennial reviews. Maintain timely and compliant documentation, and provide detailed reports on student progress.
Time Off Management for Full-Time School Employees: Employees are encouraged to schedule their time off during school breaks. Time off requests will be reviewed and approved by the supervisor to ensure minimal disruption to normal operations.
Occupational Therapist Qualifications:
Licensure: Current Occupational Therapist license in the state where services are delivered or eligibility to obtain such licensure; NBCOT registration preferred.
Clearances: DOJ/FBI Livescan Background Check and TB Clearance required; must be eligible to work in the US.
Experience: Preferred experience in school or treatment settings with students facing learning or social-emotional challenges, including involvement with IEPs.
Specialized Knowledge & Skills: Proficient in sensory integration techniques and supporting students with disabilities; certifications in Pro-Act, CPI, and/or CPR are helpful.
Communication: Excellent written and verbal communication skills in English.

Summary:
Team Rehabilitation is seeking out patient-focused Physical Therapists. Primary responsibilities are evaluating, diagnosing, and treating patients with the ultimate goal of having patients recover from injuries, manage pain, and regain mobility. The only way to drive success is in providing exceptional physical therapy with measurable outcomes. The ideal candidate will be passionate about patient care while regularly exceeding industry standards.
Responsibilities:
Provide excellent health care for our patients. Patients should achieve good outcomes and be happy with their experience of physical therapy.
Evaluate new patients - understand their current medical and emotional situations and then develop plans of care.
Treat patients and set measurable, functional goals. Making sure to answer their concerns and questions about their injury, progress and probable outcomes.
Educate the patient on treatment options and home exercises that can assist in complimenting their clinical sessions.
Supervising aides when they are providing modalities for non-Medicare, non-BCBSM patients, and also in their general set up and clean up duties.
Monitor and re-evaluate patient progress; revising plans of care and resetting goals when needed to ensure the best possible outcome.
Address pain management as a crucial component of care. Utilize techniques that can assist in pain reduction to increase their functions and everyday activities.
Determine when to discharge patients. Teach patients proper mechanics, ergonomics, and strategies to reduce future incidents.
Write timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
Achieve a reasonable level of productivity. This depends on the number of referrals and the type of patient presenting. However, with our normal outpatient orthopedic population, Team would expect 55 to 60 treatments plus evaluations a week from a full-time Physical Therapist.
Participate in research studies supported by the clinic and help develop protocols for the most common diagnoses.
Give input on policies, procedures and strategy to ensure that all Team Rehabilitation clinics provide the highest quality of care.
Handle patient feedback and complaints promptly and professionally.
Responsible for continuing to develop skills by staying updated in the latest research and techniques. Attend company in-house CEUs and use the continuing education allowance to attend seminars, workshops, and professional development opportunities.

GENERAL SUMMARY:
Under direction, provides crisis intervention and de-escalation services in the home and community. Completes hospital face-to-face screenings to determine if child needs inpatient care. Maintains clinical and administrative records. Coordinates, consults, and provides liaison work with other agencies and systems. Ensures service authorizations are current. Implements program goals and activities in keeping with program philosophy.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Maintain Children's Diversion caseload providing crisis intervention and prevention activities to help stabilize the home environment to avert hospitalization, maintain the child in their home, and assist with assimilation once a child is discharged from the hospital.
Collaborate with schools, the court system, and other organizations.

Growth Works, a long standing non-profit organization servicing the Western Wayne counties of Metro Detroit, is searching for a Residential Treatment Clinician for their Oxford, MI location. This individual provides and facilitates group therapy and treatment planning for adolescent clients with Substance Use Disorders (SUD) in a non-secure, residential facility. All services are provided in accordance with Growth Works approaches, policies and procedures. Functions in a therapeutic client-facing role where ongoing adherence to Agency policies and the ability to establish and develop appropriate client-therapist relationships based on trust, good-will and relationship building is key.
Job Summary:
This individual will provide and facilitate group therapy and treatment planning for adolescent clients with Substance Use Disorders (SUD) in a non-secure, residential facility located in Oxford Mi. All services are provided in accordance with Growth Works approaches, policies and procedures.
This individual will function in a therapeutic client-facing role where ongoing adherence to Agency policies and the ability to establish and develop appropriate client-therapist relationships based on trust, good-will and relationship building is key.
Roles and Responsibilities:
Ensures that clients are receiving the necessary clinical care to progress in completion of their treatment goals, mastering recovery, with emphasis sustained post program recovery.
Functions as a key component of the Growth Works Treatment Team, in partnership with Crossroads for Youth.
Facilitates and provides direct clinical care in group, family and individual settings while fostering a safe and inclusive treatment environment. Utilizes various therapeutic approaches, emotions regulation techniques and trauma-informed practices in keeping with Growth Works philosophy and as determined by client need.
Completion of proper documentation of the case file including, but not limited to, accurate preparation of treatment plans, discharge summaries, progress notes and biopsychosocial reports.
Attendance and participation in staff meetings, staff in-services and meetings with community partners, as assigned
Work in tandem with Crossroads for Youth to coordinate care for the clients while maintaining a healthy cross organizational partnership.
Master the Clinical Competencies to the satisfaction of the Program Manager
Addresses personal recovery matters in a manner consistent with the agency's approach to recovery.
Qualifications:
Master's degree in Social Work, Counseling, Psychology or another relevant field of study.
Current holder of Certified Advanced Addictions Counselor (CAADC) or registered development plan with MCBAP to complete certification
Current holder of State of Michigan certification as a Social Worker (LLMSW/LMSW) Licensed Professional Counselor (LLPC/LPC)
Previous experience with the SUD treatment and recovery required.
Knowledge of co-occurring disorders, trauma and brain development preferred.
Successful ability to complete agency background screenings and tests
Physical requirements:
Must be able to complete manual tasks involving dexterity to enter and retrieve data using computer systems, systems applications, and other office equipment.
Ability to lift and transport materials/supplies up to 25 lbs. for internal documentation and recordkeeping, court support, and other administrative tasks.
