Description of Responsibilities
The Home Health Physical Therapist provides skilled physical therapy services to patients in their homes on an intermittent visit basis. This role is responsible for assessing, planning, implementing, and evaluating physical therapy care to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. The therapist ensures all care is delivered in compliance with Title 22 regulations, Medicare Conditions of Participation, ACHC standards, and agency policies and procedures.

Responsibilities include the following:
Provide skilled physical therapy services to patients in the home setting, including evaluation, treatment, and patient/caregiver education.
Develop, implement, and revise individualized therapy plans of care in collaboration with the interdisciplinary team and the patient's physician.
Document all patient visits, assessments, and interventions in accordance with Medicare, Title 22, ACHC, and agency standards for accuracy, timeliness, and completeness.
Educate patients and caregivers regarding safe and effective use of equipment, exercise programs, mobility training, and strategies to maximize independence.
Monitor patient progress and communicate changes in condition or plan of care to the physician and interdisciplinary team promptly.
Adhere to all agency policies, procedures, and safety protocols while promoting quality patient care and regulatory compliance.
Participate in case conferences, staff meetings, and ongoing training as required.
Submit all clinical documentation within 48 hours of each patient visit.
Maintain professional knowledge and skills through continuing education and training in accordance with licensure and agency requirements.
Minimum Qualifications:
Current, active Physical Therapist (PT) license in the state of California.
Minimum of one (1) year recent clinical experience, preferably in home health or acute care.
Knowledge of and ability to comply with Title 22, Medicare Conditions of Participation, and ACHC standards.
Current CPR certification.
Valid California driver's license, reliable transportation, and proof of auto insurance.
Strong clinical assessment, communication, and documentation skills.
Ability to work independently while managing multiple patients and priorities.

Our Ideal Candidate: Must be able to clearly articulate and communicate theories, diagnoses, and TCM philosophies to patients, not just perform them.
Proficient in cupping, moxibustion, gua sha, and electric stimulation. Proficiency in ear seeding is a huge plus.
Capable of treating a wide range of disorders, including both physical and psychological conditions, using various techniques as well as accurately diagnosing conditions through patient interviews, observation, and interpretation of symptoms.
Be able to confidently prescribe different treatments as well as herbal formulations
Must be able to create a safe and serene environment for each patient
Evaluate and track the efficiency of treatment, conditions, and progress of each patient
Facilitate and maintain a supportive community with other acupuncturists.
Ensure proper treatment notes and client files are maintained
Adhere to treatment times to ensure scheduled appointments are maintained
Strong problem-solving capabilities and a positive, can-do attitude
Incredible at maintaining and building patient load through open and strong communication as well understanding of treatment plans.
Fluent in English and able to communicate quickly and without constraint in a way that is easily understood
Deliver a consistent guest experience that meets the VIE HEALING mission and vision
Meet patient rebooking requirements via methodical treatment plans and procedures
Fluent in VIE offerings and products, ensuring patients have access and all services, add-on and enhanced treatment options inside and outside of the treatment facility
Attend all training and staff meetings
Adhere to all VIE HEALING protocols for grooming standards, room set up, employee requirements for breaks/clocking in and out, etc. as well as specifics to execution of treatments.
Communicate effectively and respectfully with management and front-of-house staff to ensure daily client flow and operational standards are upheld
Practice timelines with clients and staff. Always arrive at least 15min prior to the first client.
Desire to expand your practice and learn.
Be open to opportunities such as corporate events and retreats

What you will do: Responsibilities listed include but not limited to:
Perform evaluations and develop treatment plans
Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
Communicate patient's needs and progress to the treatment team, physician, person receiving services and family members
Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
What you will need:
Minimum of a bachelor's degree in Occupational Therapy from a college or university with an accredited Occupational Therapy program
Current, unrestricted license as an Occupational Therapist by state in which practicing
Current CPR Certification
A minimum of one year's work experience as an Occupational Therapist
Demonstrates knowledge of rehabilitation techniques related to complex neurological diagnoses preferred
Communicates effectively and professionally in verbal and written interactions
Ability to lift 50 pounds
Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
