The Physical Therapist (PT) is responsible for examination, evaluation, diagnosis, prognosis, intervention and the outcomes of the patient/client as described in the New York State Physical Therapy Practice Act and Guide to Physical Therapy Practice (APTA). The therapist assists patients to reach their maximum performance and level of functioning, while teaching them to live within the limits of their capabilities. The PT coordinates, delegates and supervises appropriate responsibilities assigned to support staff (Physical Therapist Assistants, students, rehab aides, etc.). Interacts and practices in collaboration with a variety of interdisciplinary professionals to provide prevention and wellness services including screening and health promotion.
Core Job Responsibilities
Obtains a complete patient past medical history and history of present condition
Performs a complete system review of the patient
Completes accurate and appropriate test and measures
Determines the appropriateness for physical therapy treatment following the examination.
Recognizes and makes appropriate referral to another practitioner as needed.
Determines the physical therapy diagnosis based on the physical therapy examination
Determines the amount of time required to reach the optimal level of improvement
Develops a plan of care that specifies the interventions to be used and their timing and frequency
Conducts a reexamination to determine changes in patient/client status and to modify or redirect interventions as appropriate
Uses various physical therapy procedures and techniques to produce changes in the condition that are consistent with the diagnosis and prognosis
Observes and evaluates treatment effect, and makes changes to the plan of care as needed
Performs all aspects of patient care in an environment that optimizes patient and therapist safety and reduces the likelihood of medical/health care errors
Performs patient/ family education timely to prevent delays in discharge
Demonstrates the ability to treat patients and their families with respect and dignity.
Coordinates and directs patient care to ensure patients' needs are met and hospital policy is followed.
Determines the anticipated goals and expected outcomes for the episode of care
Participates in outcomes data collection and analysis to determine the effectiveness of intervention
Maintains accurate and timely documentation that meets current standards and policies
Interacts professionally with patients/ families and involves the patient/ family in the formation of the plan of care
Maintains current patient schedule board or book
Consults other departments as appropriate to collaborate in patient care and performance improvement activities
Education/Experience Requirements
REQUIRED:
Graduate of Bachelor of Science program in Physical Therapy with curriculum approved by the American Physical Therapy Association.
New York State Physical Therapist licensure and registration.
One-year experience as a staff physical therapist under qualified supervision.
PREFERRED
DPT or Master's degree in Physical Therapy.

The Occupational Therapist evaluates, plans and provides occupational therapy treatment programs for patients whose injury or disease interferes with daily functioning. Provide skilled intervention using varieties of techniques that are appropriate to achieve identified goals, restore function or prevent disability. This position may work in an inpatient, outpatient or home care setting.

Performs appropriate assessments on all patients (neonate, pediatric, adolescent, geriatric patients and general patient population) in accordance with patient diagnosis and physician orders.
Establishes long and short term goals and a plan of care in collaboration with patient / family / caregiver.
Implements treatment to patient according to established individualized program plan.
Uses various occupational therapy procedures and techniques to produce changes in the condition that are consistent with the diagnosis and prognosis.
Observes and evaluates treatment effect, and updates the plan of care as needed.
Formulates a discharge or follow-up plans to include continuation or discontinuation of O.T. services, implementation of home programs, recommendations for adaptations/equipment in patient's home environment, and community resources in collaboration with patient, family, caregivers and staff.
Implements an education plan based upon learning needs and evaluates for effectiveness. Includes family in teaching.
Provides positive supportive communication to enhance patients' morale and self-esteem.
Evaluates and makes recommendations regarding adaptive equipment for patients (i.e.: wheelchairs, bathroom equipment, ADL devices, etc.). Designs, fabricates, and applies orthotic devices in accordance with patients' diagnosis and physician's orders. Instructs patient and/or family in the application, care and precautions of orthosis and equipment.
Attends patient case conferences or discharge planning meetings, obtaining and reporting pertinent information regarding patient's present level of functioning, and future goals/plans.
Consults and collaborates with interdisciplinary team when needed. Makes appropriate referrals to other practitioners.
May conduct patient home evaluations via actual visits, gather information regarding home, or simulate with patient/family, and make recommendations for changes or equipment to promote patients' highest level of functioning within the home.
Ensures all documentation and reports are complete, accurate and timely. Submits all patient charges promptly.
Communicates patient or problem issues appropriately to supervisor, physician, family and staff.
Anticipates adverse outcomes or medical changes and communicates with referral source and/ or appropriate staff.
Performs all aspects of care in a manner that optimizes patient and therapist safety and reduces the likelihood of medical/health care errors.
Keeps current in trends and best practices in occupational therapy treatment techniques, modalities, equipment, and exercise programs.
Complies with all regulations, professional standards and MVHS policies.
Assists with orientation of new staff members. Provides clinical supervision to COTA and OT students.
Maintains a positive and collaborative working relationship both within the department and with other departments.
Actively participates in departmental performance improvement/Continuous Quality Improvement (CQI) activities.
Provides clinical supervision and direction to Certified Occupational Therapist Assistants, students, Rehab Aides, support staff as assigned.
Performs related duties as assigned.

DESCRIPTION
The Licensed Mental Health Clinician (LMHC, LMFT, or LCSW) will train will assist individuals, groups, and families through the counseling relationship by working through their presenting problems, defining their counseling goals, and developing action plans to resolve lifes challenges. DUTIES AND RESPONSIBILITIES
Provides client counseling services to a varied population of children, adolescents, adults, families, couples, and groups

Outpatient physical therapist job in Syracuse, NY
The After Office Hours Licensed Marriage & Family Therapist (LMFT) will provide behavioral health services for adult and/or pediatric patients followed in the primary care and other clinics at Nappi Wellness Institute. This position works collaboratively within the multidisciplinary team to provide comprehensive psycho-social management and support to patients and families. This position will provide behavioral health consultation, assessment and therapy services to at-risk patients. The LMFT will coordinate with the Integrated Care Manager and other ancillary staff on referrals to meet the psycho-social needs of patients and families. Duties include providing psychosocial consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. Assess and treat patients in an integrated care environment. Provides psychotherapy to individuals, families or in a group setting.
Minimum Qualifications:
NYS (or eligible) licensed Marriage & Family Therapist required by time of appointment. Masters degree in Marriage & Family Therapy or related field and three years of post-licensure experience providing therapy required.
Preferred Qualifications:
Previous experience as a LMFT in an ambulatory setting preferred.

There are part time 40% and full time 100% positions available. The full-time salary range for this position is $70,000-$85,000, depending on education and experience. Additional compensation has been factored into account for evening and weekend shifts.
