The Role
As an Occupational Therapist (OT), you will be responsible for evaluating residents' functional abilities and designing individualized treatment plans to improve independence in daily activities. You'll provide hands-on therapy, collaborate with interdisciplinary teams, and ensure high-quality rehabilitation services that support our residents' goals.
Key Responsibilities
· Evaluate residents' functional, cognitive, and psychosocial needs using standardized assessments.
· Develop and implement personalized treatment plans to improve daily living skills, mobility, and safety.
· Provide therapeutic interventions including adaptive equipment training, therapeutic exercise, and functional retraining.
· Educate residents, families, and staff on strategies to support rehabilitation goals.
· Document evaluations, progress notes, and discharge summaries accurately and timely in accordance with regulatory and company standards.
· Collaborate with physical therapists, speech-language pathologists, nurses, and other team members to deliver coordinated care.
· Maintain compliance with all applicable state and federal regulations.
Qualifications
· Current licensure as an Occupational Therapist (OT) in the state of practice.
· Graduation from an accredited Occupational Therapy program.
· Previous experience in long-term care, skilled nursing, or rehabilitation preferred.
· Strong communication, documentation, and organizational skills.

Schedule: This is a PRN role and requires some travel to client's homes
The Physical Therapist will provide ongoing therapy evaluations and treatments and education to clients in a timely manner per physician's orders. To provide leadership and supervision of physical therapy services to other physical therapists, assistants and aides. Possesses adequate strength and endurance to meet the physical demands involved in moving clients and equipment into position for treatment and in manipulating equipment and clients.

Be willing to travel and work at other VOANS facilities, as needed.
If operating company vehicle you must be twenty-one (21) years of age and meet the MVR organization standards.
Maintain productivity standards as directed by supervisor.

ESSENTIAL FUNCTIONS:

Advanced Clinical Assessment & Diagnosis:

A Therapist at Catholic Charities:
SUMMARY
Provides professional counseling, clinical assessment/problem identification, treatment planning, case management and crisis intervention to adults, adolescents and children, couples and families according to program standards and procedures and consistent with Catholic teachings.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Obtains information about clients through use of interview, case history, and observational techniques.
Evaluates data to identify causes of problem of clients and to determine advisability of counseling or referral to other specialists or institutions.
Conducts counseling or therapeutic treatments, in either individual or group sessions as appropriate, to assist clients in gaining insight into personal problems, define goals, and plan actions reflecting interests, abilities, and needs and to discuss progress towards achieving goals.
Follows up results of counseling to determine effectiveness of treatment used.
Consults with medical doctor or other specialists concerning treatment plan and amends plan as directed.
Refers client to supportive services to supplement treatment and counseling.
Completes necessary administrative duties (paperwork, etc.) timely and accurately.
Acts as an advocate for clients and/or client systems when appropriate.
