Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions and we are currently looking for a PhysicalTherapist (PT) to join our dynamic home health care team. In this position, you make it possible for patients to recuperate from illness or injury in the comfort of their own home.
Territory: Eau Claire communities
Schedule: Part time Visits any day Monday - Friday
Essential Job Functions:
Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
Responsible for administration, evaluation, and interpretation of tests and measurements to determine functional goals and outcomes.
Provide a written evaluation and plan of care defining physical therapy needs, goals, progress and expected outcomes.
Delivery of physical therapy services to comply with prescribed physician orders.
Maintains plan of care by performing timely documentation of client performance, modifying goals and treatment as needed, and determine discharge care plans as appropriate.
Responsible for discharge planning, including collaborating and educating the patient and his/her caretakers on home exercise plan to follow discharge.
Participates in coordination of services with health care team, physicians, parent/guardians, nurses, etc.
Documents according to policy and procedure and requires minimal supervision or instruction.
Documents effectively resulting in no technical or clinical denials subsequent to review of documentation by payers
Maintain effective communication between staff, healthcare team members and family. Serve as a resource consultant for clinical staff.
Supervises other personnel (PTA, HHA) as applicable.
Completes OASIS assessments where allowed by state professional practice and regulation.
Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
Maintain effective communication between staff, healthcare team members and family. Serve as a resource consultant for clinical staff.
Supervises other personnel (OTA, HHA) as applicable.
Completes OASIS assessments where allowed by state professional practice and regulation.
Requirements:
Degree in Physical Therapy from an accredited university/program.
Current, unrestricted state license as a PhysicalTherapist in the state of practice
Valid CPR and Drivers License
Preferred:
* Experience in Home Health Therapy and completion of OASIS documentation

**Now Hiring: Travel PhysicalTherapist - Outpatient Clinics**
Schedule: Day shift | 30-40 hours per week
Duration: 13 weeks
**_Important: Must reside 50 or more miles from clinic location_**
**Key Responsibilities:**
+ Perform comprehensive physical therapy evaluations, including medical history reviews, functional assessments, and diagnostic testing
+ Provide skilled therapeutic interventions, including manual therapy, therapeutic exercise, neuromuscular re‐education, gait training, and modalities as appropriate
+ Communicate regularly with referring providers regarding patient progress, treatment updates, and clinical recommendations.
+ Provide patient and caregiver education on home exercise programs, injury prevention, and recovery strategies to support long-term outcomes
**Qualifications:**
+ Current MN state licensure as a PhysicalTherapist is required
+ Current AHA BLS is required
+ Experience in Outpatient orthopedic or sports medicine settings is preferred
**Pay Breakdown:**
+ $700 per week (taxable)
+ $1,526 weekly per diem (non-taxable)
**Pay Details:** $2,226.00 per week

The Licensed Mental Health Therapist is a key part of our integrated Care Team, helping drive the creation and implementation of our tech-enabled psychotherapeutic care for our members. At Reema, we believe integrating social and clinical care is the most effective way to support member wellbeing. You'll care for members in an outpatient clinical setting, primarily via telehealth, but also in the home and community as needed. You will be part of a multidisciplinary team made up of Psychiatric Nurse Practitioners and non-clinical Community Guides to collaboratively provide a holistic approach to member health, and support you to work at the top of your license. Together, this team is responsible for providing and coordinating member-centered quality care for a panel of members with complex health and social needs.
Reema is expanding our service delivery into behavioral healthcare, so we are looking for someone who is comfortable helping create the details of our care model within the ambiguity of a fast-paced startup. For the right person, this position has the potential to grow into a leadership role.
Primary Responsibilities:
Assess and diagnose behavioral health conditions, both chronic and acute.
Formulate treatment plans for members in consultation with other members of the collaborative health team as the member's condition warrants.
Ensure a behavioral health perspective is incorporated into comprehensive care planning for complex patients.
Complete initial intakes and assess patient risks and needs, delivering therapeutic interventions and therapy, and referring and navigating to community resources for access to the broader behavioral health continuum of care.
Collaborate with primary care providers, psychiatrists, social workers, or other professionals to discuss treatment plans and progress.
Provide psychoeducation to members, emphasizing mental health promotion and disease prevention
Support referral for members who need additional specialty behavioral health care (e.g. to an in-patient facility) and advise on treatment plans until members are engaged in appropriate care.
Establish and foster trusting relationships with patients and ensure care is aligned with the patient's goals and values.
Participate in multi-disciplinary case conferences to ensure holistic and appropriate care for patients.
Provide formal and informal coaching and education to other clinical and non-clinical team members about supporting patients with behavioral health issues.
Support post-discharge planning to prevent readmissions.
Timely and appropriate documentation in our EMR to manage member care and coordination with the multidisciplinary team.
Contribute to the evolving design and implementation of clinical pathways and protocols for therapeutic treatment of behavioral health conditions in Reema's care model to ensure we are providing the best patient care possible.

Experience and Skills:
4+ years of outpatient experience working with adults enrolled in Medicaid, Medicare, or are dual-eligible.
3+ years of experience in assessing, planning, and managing behavioral patient care as acquired through direct clinical experience, including direct therapy and care/case management work
3+ years of experience working with individuals who have significant mental and chemical health needs and believe that recovery is possible.
1+ years of experience practicing telemedicine
Experience with underserved populations facing socioeconomic barriers to health care
Excellent working knowledge of trauma-informed and evidence-based psychotherapy treatment for mental and chemical health conditions
Awareness and comfort in participating in an integrated behavioral health model where you will work alongside Community Guide team members
Comfort with electronic medical record documentation and excited about how technology can support your work and drive ongoing improvement towards new and better care
Ability to make quick and accurate clinical assessments of mental and behavioral health conditions
Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent, and equitable manner
Ability to work effectively as a team, interfacing with patients, primary care providers, consulting psychiatrists and mental health specialists, as well as with administrative and support staff
Working knowledge of all related computer and software applications such as Microsoft Word, Outlook and Excel, telecommunication and Video Conferencing required; G-Suite preferred
Education, Qualifications & Position Requirements:
LCSW, LMFT or LPC with an active license in the state of Wisconsin
Willingness to cross license in Michigan, Kansas and Missouri
18 years of age or older

JOB SUMMARY: To provide outpatient mental health services to the citizens of Winona, Houston, Fillmore, Wabasha, Goodhue and other participating County partners, in a non-profit community mental health clinic setting with other licensed professionals and Center staff including working with individuals, couples, families and the community at large.
JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:
Assess and document client's current life situation, including reasons for referral, history of the current mental health problem including important developmental incidents, strengths, and vulnerabilities, current functioning and symptoms, diagnosis using the DSM 5 or other updated tools, and providing needed mental health services.
Develop a written plan of intervention, treatment and services identifying goals and objectives of treatment, treatment strategy, a schedule for accomplishing treatment goals and objectives, and the individual or individuals responsible for providing treatment to the person.
Perform planned and structured treatment (individual, family and/or group psychotherapy) based on client's diagnosis.
Assist in the coordination of therapeutic services with other Center staff, including consulting with on-staff psychiatric prescribers.
Consult in a timely manner with other professionals or agencies concerning clients referred to or from the Center as well as mutual clients.
Participate in the required multi-disciplinary team meetings and clinical supervision.
Provide case or program consultation to health care and human service professionals and agencies.
Participate in efforts to coordinate agency and community resources.
Participate in conferences, seminars, or other educational activities based on the annual staff development plan and as needed to be in good standing with state licensure requirements.
Provide training to in-house staff, share approved and legal video/audio tapes of sessions with clients for intra-agency training or as requested by the Clinical Director.
Perform other outpatient mental health activities such as giving presentations at all-agency meetings or to area businesses, schools, and other organizations as directed by the Director or staffing information booths at events or fairs and participating in activities outside of the agency as directed.
Maintain accurate and current documentation of all client treatment activities in the agency's ECR. Progress Notes are to be completed within 48 hours of seeing a client; treatment plans completed at the first therapy session; diagnostic assessment completed within insurance compliance guidelines or sooner in time sensitive situations and as requested by county or other partner staff.
Maintain accurate and up-to-date case closings and terminations, typically within three months of the most recent session with a client.
Maintain a thorough and accurate schedule in the electronic health record, including all productive and non-productive time and activities.
Maintain productivity time standards - billable and non-billable at the required minimal level, currently 65% of total paid time.
Complete all information/forms needed by the business office (Enrollment form, prior authorizations, requests for information or letters sent to doctors etc.) in a timely manner.
Develop and maintain good working relationships with all referral sources and intra-agency staff.

Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities and exercise. Promotes patient safety by participating in safety initiatives as requested and appropriately follows all safety and infection control measures pertaining to the general working environment.
Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care. Pursues professional growth through participation in continuing education courses, department in-services, and reading professional literature. May supervise physical therapy assistants following all applicable state and local compliance requirements. Includes delegating patients to assistive personnel while providing direction, recommendations regarding treatment and ongoing supervision while maintaining responsibility for patient plan of care and goals. May supervise physical therapy extenders (rehab aides/techs) in administering treatments to patients and will teach them appropriate techniques to perform with specific patients. The worker will travel to Barron, Chippewa, Dunn, Eau Claire, Pierce, Polk, Price, Rusk, and the surrounding area to meet with clients. This position has the potential to increase to full-time. This worker will:
Provide psychoeducation and psychotherapy (if qualified) to children and adults referred by CCS (Comprehensive Community Services) in their home, school, or other community-based settings.
Form trusting relationships, primarily with clients, and also with relevant family members and team members.
Provide psychoeducation about trauma and attachment disruptions, normalizing experiences, i.e., ‘It's not what wrong with you, it's what happened to you'.
Model and teach emotional regulation skills.
Work effectively as part of a Wraparound or Recovery Team to assist clients to lead a more self-directed recovery journey.
This position requires that the ideal candidate will have excellent self-awareness, excellent communication skills, a strong ability to work independently and build relationships, significant computer skills, attention to detail; and the drive and passion to patiently and creatively help clients heal from the effects of trauma, even while understanding that those effects may make the client push away the very people who want to help the most.
