Responsibilities:
Perform initial and ongoing physical therapy evaluations, including short and long-term goals, collaboration demands of other staff members, and required patient and caregiver involvement. Implement Plan of Treatment on a consistent schedule and adjust this schedule and plan according to patient progress.
Document patient progress according to approved medical documentation and facility documentation procedures.
Report patient progress to the patient, patient caregivers and loved ones, facility staff, and healthcare professionals involved in ongoing treatment/care of patients.
Ideal Candidate:
Proven record of matching patient needs to treatment plans and proven record of implementing these plans safely and effectively.
Experience assisting and directing treatments given by aides, technicians, and assistants.
Experience with recommending assistive equipment to patients and outpatient or home health follow-up programs.
Experience with a wide range of patient age groups and injuries.

Qualifications:

Under clinical supervision, the Mental Health Clinician I, provides mental health direct services in an outpatient setting that includes individual therapy, intake assessments, and facilitating skills classes/groups for the Recovery Oriented level of care. Direct services are provided for adult consumers of the Kings County area, including some travel to Avenal, Corcoran, or Kettleman City.
How will I contribute
Provide direct services to individuals assigned to caseload.
Evaluate consumers for serious psychiatric problems and provide appropriate treatment plans and carry out treatment using a wide variety of treatment modalities such as individual, group, conjoint and family therapy.
Maintain a monthly productivity standard of 100 hours of billable time per month.
Participate in Access Rotation schedule as directed.

Acts as the liaison between the program and family/community agencies including individuals such as Parents/Caregivers, Case Managers, County Social Workers, Teachers/Education Providers, and Probation Officers. The Clinician shall work with the family or care providers in the implementation of treatment plans and will maintain contacts with all important individuals in the client's life.
Provides individual, family, and group counseling based on the needs of the clients served.
Meets Agency service percentage standard reflecting expected time spent with clients, if applicable.
Completes other assigned program operational tasks in accordance with supervisory directives.
MINIMUM REQUIRED EDUCATION AND EXPERIENCE
Completion of a clinical program with a Master's or Ph.D. degree in a field of behavioral science from an accredited educational institution.
Holds a current license registration issued by the Board of Behavioral Sciences (AMFT, ACSW, APCC, LMFT, LCSW, LPCC)

2. Medication Support & Health Monitoring
Administer medications and treatments following the physician's prescriptions.
Issue medications from the dispensary and maintain records in accordance with TPEG procedures.
Reports symptoms, reactions, and progress of patients to the Nurse Practitioner and/or Facility manager, and appropriate action has been taken.
Take and record measures of the patient's physical condition, using devices such as thermometers and blood pressure gauges.
Monitor patients' physical and emotional well-being and report unusual behavior or physical ailments to the staff.
Documents nursing observations.
3. Nutrition & Meal Preparation
Inventories food, labels and dates opened food items, labels food with expiration dates, organizes pantry and fridge, prepares grocery order, takes water and fridge/freezer temps.
Ensures that patients receive appropriate nutrition, including shopping for groceries, preparing meals.
4. Documentation & Communication
Assist in maintaining patient records by reviewing case notes and documenting progress.
Communicate patient progress by participating in interdisciplinary meetings and daily briefings.
5. Professional Development & Improvement
Initiates change to improve patient care; discusses changes with the doctor when appropriate.
Utilizes educational opportunities within the facility and other avenues to maintain clinical expertise to promote personal growth and development.

Our Therapists will be working with adults, adolescents, children and families, primarily in groups, with some individual and family work.
