Evaluates, fits, and adjusts prosthetic and orthotic devices and recommend modifications.
Identifies and documents goals, anticipated progress and plans for reevaluation.
Qualifications and requirements:
Degree from an accredited Physical Therapy program
PhysicalTherapist License in State
Basic Life Support (BLS) obtain within 30 days of hire
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment and take appropriate action.

Qualifications
Master's Degree from an accredited college or university, in Social Work or a clinical related mental health field required.
Must possess a TX State licensure: LPC, LPC-I, LPC-A, LMSW, LMFT or LCSW required.

Education:

How you'll contribute:
The PHP Therapist is responsible for delivering therapeutic services to individuals in the Partial Hospitalization Program, including group and individual therapy, discharge planning, and communication with interdisciplinary and external stakeholders. Demonstrate knowledge of confidentiality, HIPAA, and clinical regulations (TJC, CARF, Medicaid, etc.).
Assist care coordination with discharge planning and continuity of care.
Complete all documentation thoroughly, accurately, and within facility timelines.
Perform additional duties as assigned by leadership.

RESPONSIBILITIES
