About the Position
We have an opportunity for an OutpatientTherapist to provide clinical services at our Downtown Clinic in Medford, Oregon. The OutpatientTherapist reports to the Outpatient Clinical Manager for day-to-day supervision and work assignments. In this position, you are responsible for performing a Provisional Assessment for individuals experiencing crisis. The OutpatientTherapist will also provide clinical oversight and consultation to facility staff and liaison with other community providers. In addition, you will receive crisis de-escalation training, which requires performing various physical, hands-on maneuvers and techniques. If desired, the OutpatientTherapist will be eligible to participate in our supervision toward licensure program after their successful completion of the 90-day introductory period.
Work Schedule: Monday through Friday, 8:00am to 5:00pm (Full Time, Day)

Additional 5% Language Differential offered for Bilingual or Multilingual candidates.

About the Program
Our Outpatient Program provides assessment and evaluation, treatment plan development, and provides clinically indicated outpatient therapy, case management, skills training, and psychiatric (medication) services. The Psychiatric Services Team operates as a branch within the Outpatient Program, specializing in psychiatric, nursing, and medication services. Psychiatric services are provided both on-site and remotely through telemedicine.
ColumbiaCare Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program.
What You'll Need
Must be eligible for Qualified Mental Health Professional (QMHP) registration. Eligibility requires:
Master's Degree in Psychology, Counseling, Social Work, or related field Unlicensed therapists MUST be actively seeking or willing to obtain licensure.TWO (2) YEARS of post-graduate clinical experience in a mental health treatment setting is preferred
If not already registered as a QMHP through the Mental Health & Addiction Certification Board of Oregon (MHACBO), we will assist you in registering within 30 days of hire.
Successful applicants must have the ability to pass a DHS criminal background check.
Physical Requirements: This position requires the ability to frequently sit, talk, listen, and use hands and fingers. It may require the ability to occasionally walk, climb stairs and ladders, bend, stoop, squat/kneel, and perform other physical tasks as applicable; as well as lift, carry, push, and pull up to 20 pounds. I. Position Summary

* Completes comprehensive mental health, crisis, and risk assessments. Develops clinical formulations, diagnoses individuals, and identifies medical necessity for treatment.
* Develops, coordinates, and/or oversees the implementation of client treatment plans with measurable objectives; as appropriate, makes referrals to other services or community agencies to meet client need.
* Provides specialized individual, family, and/or group counseling and therapy, education, outreach, and support to clients and families as necessary.
* As part of a multi-disciplinary team, provides evidence-based and fidelity models of care specific to program assignment.
* Provides consultation, education, information, and resource advocacy to agencies in the community which routinely come in contact with clients; maintains liaison with key community agencies that serve people with mental illness.
* Prepares and maintains required records, progress notes, and files; prepares reports as needed.

About the Position
We have an exciting career opportunity for an Intensive Supportive Housing Therapist who possesses a combination of skills and QMHP eligibility to provide clinical services for our Southern Oregon supportive housing programs in Medford, Oregon. The Intensive Supportive Housing Therapist position requires the ability to conduct mental status examinations and DSM-5 diagnoses. They will conduct comprehensive psychosocial mental health assessments and complete assessment reviews and updates every six months or as clinically indicated. The Intensive Supportive Housing Therapist will perform certain mental health functions, such as consultation, crisis management, and individual and group therapy, or delegate these functions to appropriately qualified clinical staff. What You'll Need
Must be eligible for Qualified Mental Health Professional (QMHP) registration. Eligibility requires:
Master's Degree in Psychology, Counseling, Social Work, or related field Unlicensed Therapists MUST be actively seeking or willing to obtain licensure.
If not already registered as a QMHP through the Mental Health & Addiction Certification Board of Oregon (MHACBO), we will assist you in registering within the first week of employment.
Successful applicants must have the ability to pass a DHS criminal background check.
Physical Requirements: This position requires the ability to frequently sit, talk, listen, and use hands and fingers. It may require the ability to occasionally walk, climb stairs and ladders, bend, stoop, squat/kneel, and perform other physical tasks as applicable; as well as lift, carry, push, and pull up to 20 pounds. Summary:
The PhysicalTherapist evaluates, diagnoses, and treats patients with physical impairments to improve mobility, reduce pain, and enhance quality of life. Essential Work Functions:
Evaluate patients' physical conditions using standardized tests, clinical examinations, and patient history to identify functional limitations and treatment need
Develop personalized rehabilitation programs based on assessment findings and patient goals
Implement physical therapy treatment plans using various techniques to improve patient mobility and reduce pain
Operate specialized physical therapy equipment and modalities safely and effectively, ensuring proper maintenance and calibration of devices
Assess patient progress, adjust treatment plans as necessary, and accurately document evaluations, interventions, and outcomes in the medical record
Work closely with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated care and optimize treatment outcomes
Educate patients and caregivers on home exercise programs, proper body mechanics, injury prevention strategies, and self-management techniques to promote long-term functional recovery
Maintain a safe treatment environment by adhering to infection control protocols, safety guidelines, and regulatory standards while addressing patients' comfort needs during therapy sessions
Engage in quality assurance initiatives and professional development activities to remain current with best practices in physical therapy
Collaborate in discharge planning to ensure smooth transitions and continued patient progress in community or home-based settings
Perform other duties as assigned within scope of practice
Required Essential Skills:

The ability to read, write, and communicate in the English language
Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
