About the Role
Lyra developed an innovative video therapy program called Lyra Care Therapy: blending live video sessions with between-session digital psychoeducation and customizable skill building tools. With this program, you'll have the resources and support you need to ensure that your clients get better-all while improving access to mental health care from your home or preferred setting.
Daily: As a Lyra Care Therapist, you'll provide short-term, evidence-based treatment via live video, maintaining a caseload of diverse and varied clinical needs. You'll assign digital lessons, videos, and assessments to your clients to enhance learnings and monitor outcomes between sessions. The anticipated starting base salary for a full-time Licensed Mental Health Therapist at Lyra is between $78,000 annually. The base salary is determined by role and placement within the range, and will depend on a number of job-related factors, including but not limited to your skills, qualifications, and location.

MAJOR RESPONSIBILITIES
Administers physical therapy evaluations. Participates in the development of the Plan of Care.
Provides physical therapy treatments, procedures, evaluations and diagnostic tests for clients under the direction of their physician for whom therapy has been medically prescribed.
Translates all exercises into functional activities or activities of daily living.
Provides instructions in the use and care of assistive devices.
Develops needed plans for modifying equipment, appliances and the physical surroundings in client's homes.
Prepares clinical and progress notes. Reports to physician and the Case Manager the client's response to treatment or change in condition.

Participates in Case Conferences as indicated.

Assists individuals and families to accept and adjust positively to physical, mental and social limitations.
Participates in the development of community resources to meet the needs of clients.
Maintains positive and effective communication with all staff and others. Understands and participates in team concept.
Maintains absolute confidentiality of all information pertaining to clients, families and employees.
Maintains a safe client environment and identifies and reports to the office any suspected vulnerable client abuse, neglect, or financial exploitation.

PHYSICAL/ENVIRONMENTAL DEMANDS
Heavy to Very Heavy. Involves lifting clients, bending, stooping, stretching, and setting up equipment. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather.
Exerting in excess of 100 pounds of force occasionally, in excess of 50 pounds of force frequently, and in excess of 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for heavy work.
QUALIFICATIONS
Graduate of an accredited Physical Therapy program; currently licensed as a PhysicalTherapist in the state of Minnesota. The license must not have been revoked, suspended, and without limitations or restrictions.
Have at least two (2) years experience in a health care setting or equivalent experience. Home Health Care experience preferred.
Strong interpersonal communication and teaching skills.
Strong organizational skills and the ability to work independently.
Excellent written and oral communication skills.
Access to a dependable vehicle or public transportation in order to travel to multiple company business stops a day.
A valid driver's license and proof of car insurance when using a personal vehicle for company business.
Proof of negative mantoux or documentation of negative chest x-ray.
Current CPR certification is highly recommended.
Have U.S. Citizenship or evidence of valid Alien Work Permit.

Turns MD verbal orders into office within 24 hours of receipt for processing and mailing.

Determines frequency of visits for patients and receives minimal recommendations for changes from supervisor when reviewed.

We are seeking a CHT (Certified Hand Therapist) credentialed casual occupational therapist to provide hand therapy services with our Rehab Flexible Workforce team! The Hand Therapist is responsible for providing safe and effective delivery of patient care including Examination, Evaluation, Diagnosis, Planning, Intervention and Establishing Outcomes.
+ Casual position; authorized to work up to 40 hours per week.
+ Position may provide coverage for various locations across the Twin Cities metro area; including, Blaine, Burnsville, Edina, Eagan, Elk River, Maple Grove, Minneapolis, Princeton, Woodbury, and Wyoming.
Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

**Responsibilities**

**Required Qualifications**

+ Basic Life Support (American Heart Association or Red Cross)
**Preferred Qualifications**
+ Master's Degree in Occupational Therapy
+ 1+ years experience in Hand Therapy
